SUBJECT:
SUMMARY
OF MEETING ON PALO VERDE, UNIT 3, DIESEL GENERATOR FAILURE A meeting was held in Bethesda, Maryland on January 20, 1987, with representa-tives of the licensee.
The purpose of the meeting was to discuss the failure, on December 23, 1986, of the 3B Emergency Diesel Generator on Palo Verde, Unit 3, and corrective actions.
Enclosure 1 lists the meeting attendees which included NRC technical staff and consultants.
The meeting is summarized as follows:
SUMMARY
The licensee's presentation was based on the viewgraphs provided as Enclosure
- 2. The licensee's conclusion was that the failure of the 89 MASTER ROD was caused by the use of crack-prone iron plating for repair of overmachined surfaces during manufacture.
The master rod fracture mode was high cycle fatigue, with very low overstress.
Part 21 reports are being filed by the licensee and by the diesel manufacturer.
All rods with iron plating in high stress areas have been removed (connecting rod cylinder number 9 in diesel 2A, and cylinder number 2 in diesel 3B).
One nickel spray plated rod was removed from the'A diesel and replaced due to some slight disbonding of the repair material.
The licensee agreed to ultrasonically inspect the three nickel spray repaired rods on the 2B diesel.
This technique appeared to detect cracking on the 3B diesel inspections.
Based upon the briefing, the staff feels that the investigation has been well organized, involved appropriately qualified personnel of the licensee, its contractors and the diesel manufacturer.
Final staff judgment will be reserved on the root cause and repair until the the final report is received.
