The

following violation

was

identified

during

an

inspection

conducted

on

February

12 - March 11,

1985.

The Severity Level

was assigned

in accordance

with

the

NRC Enforcement Policy ( 10

CFR .Part 2, Appendix C).

Unit

specified

in Table 3.6-2 shall

be operable

in Modes

1 through

4 or

various actions

be taken within 4 to

to correct

the situation

or

place the unit in cold shutdown.

Additionally, operating

procedure

0010129,

Rev.

5 requires that Class

I equipment

being

removed

from service

be listed

in the unit's equipment out-'of-service

log.

Contrary to the above,

valve HCV-14-7,

a containment isolation valve in the

component

cooling water

supply to the reactor

coolant

pumps,

was mechani-

cally jumpered

"open"

on December

31,

1984,

rendering

the valve inoperable.

This condition existed until February

28,

1985,

when the jumper was

removed.

The jumpered valve was not- listed in the equipment out-of-service

log.

This is

a Severity

Level

IV violation (Supplement I).

Pursuant

to

10

CFR 2.201,

you are

required

to submit to this office within 30

days of the date of this Notice,

a written statement

or explanation

in reply,

including:

(1) admission

or denial

of the alleged violations; (2) the

reasons

for the violations if admitted;

(3) the corrective

steps

which have

been

taken

and

the results

achieved;

(4) corrective

steps

which will be

taken

to avoid

further violations;

and (5) the date

when full compliance will be achieved.

Security

or

safeguards

information

should

be

submitted

as

an

enclosure

to

facilitate withholding it from public disclosure

as required

by 10

CFR 2.790(d)

or

10

CFR 73.21.
