NPF-41 This special report is required by Palo Verde Unit 1 Technical Specification 3.3.3.1, Table 3.3-6, Action 27 for a Radiation Monitor which was inoperable for greater than 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br />.
At approximately 0436 MST on April 19,
- 1987, Palo Verde*Unit 1 was in Mode 1
(POWER OPERATION) operating at 100 percent power when a Containment Building Area Radiation Monitor (RU-149) was declared inoperable due to spurious detector failure alarms.
As required, a preplanned alternate program to monitor the appropriate parameters was initiated.
Troubleshooting to determine the cause of the alarms was commenced under an approved work order.
During the course of troubleshooting, the signal cable connector at the microprocessor was cleaned
- however, a cause for the spurious alarms could not be determined.
The radiation monitor was put back on line on April 21,
- 1987, but was maintained in an inoperable status to allow for additional troubleshooting and monitoring for approximately seven days.
No failures or abnormalities were detected during this time period.
Because of the inability to recreate the spurious alarms and the inability to isolate a particular deficiency, no root cause could be identified.
The appropriate surveillance testing was conducted an'd RU-149 was restored to an operable status on April 28, 1987 at 0875.
Should RU-149 exhibit similar problems, further troubleshooting will be conducted to try and determine a root cause.
