3.3.3.3 ACTION Statement a and T.S. 6.9.2.

At approximately 0215 MST on April 8, 1987, Palo Verde Unit 1 was in Mode 1 (POWER OPERATION) at 100 percent power when the 30 day ACTION Statement for operability of the seismic monitoring system was exceeded. The seismic monitoring system had been declared inoperable at approximately 0215 MST on March 9, 1987 due to the malfunction of the Channel 3 data recorder. The data 'recorder malfunction occurred during the performance of 41ST-1SM01 (Seismic Instrumentation Channel Checks 4.3.3.3.1).

Troubleshooting conducted under an authorized work document identified a defective logic card and a circuit breaker that was open or not making proper contact. The circuit breaker is located upstream of a negative 12 volt power. supply. The logic card was replaced and the circuit breaker was reset. A root cause of failure analysis has been initiated to determine the cause of the logic card malfunction.

On April 7, 1987, the performance of 36ST-1SM01 (Seismic Monitoring System Functional Test Procedure) and the appropriate portions of 36ST-1SM02 (Seismic Monitoring System Calibration Test) were initiated to return the seismic monitoring system =to operable status . However, during the performance of the surveillance tests, vertical indication could not be obtained from the Channel 2 data recorder. The vertical indication is a representation of the vertical motion that would be experienced during an earthquake.

Troubleshooting determined that a ribbon cable wire had separated from a connector.

The ribbon cable connects the Channel 2 data recorder with surveillance test equipment.

This ribbon cable is permanently installed. The cable was repaired and the performance of 36ST-1SM01 and 36ST-1SM02 was resumed. Following the successful completion of 36ST-1SMOl, 36ST-lSM02, and 41ST-1SM01 the Seismic Monitoring System was declared operable at 1545 MST on April 9, 1987.
