1-SR-87-011 This Special Report is being submitted pursuant to Technical Specification (T.S.)
3.3.3.9 ACTION 42b and T.S. 6.9.2 to report an event in which a high range noble gas activity monitor (RU-144) was inoperable for greater than 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br />.
The 72 hour8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br /> limit for operability was exceeded at approximately 0837 MST on February 28, 1987.
Pursuant to T.S. 3.3.3.9 ACTION 42a the Preplanned Alternate Sampling Program was initiated to monitor the plant ventilation system.
At approximately 0837 MST on February 25,
- 1987, Palo Verde Unit 1 was in Mode 5 (COLD SHUTDOWN) for a steam generator tube leak outage when RU-144 was taken out of service for testing of the associated Plant Vent Low Range Monitor RU-143.
No physical testing
- was, or required to be performed on RU-144.
During surveillance
- testing, prior to placing the monitor back in service, it was discovered that the check source installed on -one of three Particulate Iodine Samplers detector did not meet the acceptance criteria of the Functional Test.
The check source portion of the functional test being performed was not 'required per T.S. 4.3.3.9 for the Particulate Iodine samplers.
- However, the check source is activated and the before and after levels are compared for proper response of the detector to radioactive material.
This is performed for all monitors.
Troubleshooting conducted under an authorized work document revealed that the check source was not actuating.
As a corrective action, the check source was reworked by realigning the rod to complete a full actuation travel path to meet original requirements.
A root cause of failure request has been initiated and will address the potential of similar cases in other monitors encompassing all three units.
To return RU-144 to service, Quarterly Functional Test (36ST-9SQ04) was successfully performed.
The monitor was declared operable at 0855 on March 6, 1987.
The monitor was unavailable for 9 days and 18 minutes.
