Dear Sir:
The referenced letter transmitted our report (Rev.
1) concerning the single pipe support failure in the Unit 1 containment.
We have continued to evaluate this incident, and we have determined that eccentric loading of the support was also a significant factor in the cause of the failure.
The updated report (Rev.
- 2) is attached 'to this letter.
Evaluation of eccentric loading resulted in the identification of three supports requiring design modifications.
Necessary modifications will be completed prior to entering MODE 2.
