SMA Welding in the CEA Shroud.
Dear Mr. Knighton:
During a telephone conversation, November 9, 1984, held between
- APS, C-E and the NRC to discuss the information provided in the referenced letter, Joe Halapatz, of your staff, had a question concerning the disposition of a NDE exam performed on the CEA Shroud Top Plate Welds.
The particular concern was that there was no documentation available to indicate that any rework had been performed which could have had an effect on the results of the reexamination.
After a discussion of the procedures employed in the NDE examination, C-E stated that the nonrelevant indication had been reexamined following a local surface conditioning, which is a normal acceptable practice.
The reexamination included a documented LP and a visual exam report.
C-E then referred Mr. Halapatz to the ASME Boiler and Pressure Vessel
- Code, subsection NG, subarticle NG-5000, subparagraph NG-5351 (a) and (b) which stated that, "...localized imperfections such as may occur from machining marks or surface conditions may produce similar indications which are nonrelevant to the detection of unacceptable discontinuities.
(b) Any indication which is believed to be nonrelevant shall be regarded as a
defect and shall be reexamined to verify whether or not actual defects are present.
Surface conditioning may precede the reexamination."
In compliance with the Code, C-E had reexamined the area after a surface conditioning operation.
The surface conditioning which preceded the second exam was a grinding operation classified as cosmetic.
No record of the details of the cosmetic repair operation was available, because none are required for such operations.
Although no evidence was available to testify that any work had preceded the reexamination, normal procedure would call for such conditioning prior to the reexamination, Bo~>isoo6i"8~< sio t

and the NDE report would give testimony to that effect.
C-E then read the Inspection Service Report Log 819456 which provided the detailed record of the sequence of events.
Mr. Halapatz, at this time, acknowledged that this explanation addressed his concern on the CEA Shroud Plate Weld.
