Site Visit Agenda August 22 and 23, 1984 1.

Tour of all known exit points for each leak 2.

Observe condition of fill in (a) areas where saturated by leakage and (b) undisturbed areas 3.

Discussions with people familiar with design, construction, leakage

problems, foundation conditions, and grouting 4.

Documents requested for discussion include:

(a) direct statements or reports from the people that reported the deficiency (b) as built geologic and soils maps (c) foundation reports describing geology, soils, and foundation design 5.

Specific topics for discussion include the following:

(a)

In response staff question 3, a statement is made that only one exit point occurred for leakage (into the seismic gap).

Where was the seepage noted on the surface?

(b)

During the grouting program, it is noted that the largest excess grout take occurred at Unit 3.

No eroded fill materials were observed at the reported exit point for this leak.

What are the potential consequences of undetected erosion of fill in this and other areas'c)

No discussion'as provided regarding settlement of equipment hatch or potential loss of support under this structure or the adjacent wing of the auxiliary building and other nearby structures.

Explain why this was not necessary considering the leakage that exited into the settlement hatch and the possibilities of undetected erosion.
