Darrell G. Eisenhut, Director Division of Licensing Office of Nuclear Reactor Regulation EVALUATION OF FAN BLADE POTENTIAL MISSILE HAZARDS -
PALO VERDE On September 24, 1982, Arizona Public Service Company reported to Region V
that a fan assembly in the normal air conditioning system of the Palo Verde Unit 1 containment had failed, resulting in a fan blade passing through the fan housing and impinging on the containment liner.
As a result, by memorandum dated October 1, 1982, Region V requested that NRR assume lead responsibility in evaluating whether fan assembly failures have been adequately considered in missile protection analysis.
In order to perform this evaluation, NRR requested APS to provide additional information regarding the cause of the fan blade failure and to address whether additional measures are required to protect against such failures.
The applicant's response was provided by letters dated September 1 and 26, 1983.
The applicant stated that the cause of failure was due to undertorquing of the blade attachments and that measures are being taken at Palo Verde to assure that the blades are properly torqued.
The applicant also performed an evalua-tion to demonstrate that if such fan failures were to recur at Palo Verde, they would result in no adverse consequences.
The staff's evaluation of the applicant's response is included in Section 3.5 of Supplement No.
5 to the Palo Verde SER, dated November 1983.
In Supplement No. 5, the staff concludes that adequate protection is provided against fan blade missile effects.
Since Joy Manufacturing fans are used at other nuclear facilities, NRR also considered the generic implications.
As a result, by memorandum dated November 4, 1983 to E. L. Jordan, NRR provided IE with a proposed IE Notice to be sent to all licensees and applicants to assure that fan blades/attach-ments are properly torqued in accordance with manufacturer's recommendations.

IE has determined that INPO recently issued a Significant Event Report (SER 63-83, dated September 9, 1983) which deals with the fan blade failure at Palo Verde, as well as one at LaSalle.
Consequently we have concluded that further generic correspondence on this matter is not warranted at this time.
This completes the action on your request of October 1, 1982, and we consider NRR actions on TIA 82-62 complete.
