This letter refers to the Inspection conducted by Messrs.

W. Bishop on August 17 21, 1981, as documented in your letter of, November 19, 1981, of activities authorized by the Nuclear Regulatory Commission (NRC) Construction Permit Nos.

CPPR-141, 142 and 143.

During this Inspection, three (3) items of noncompliance were identified.

Our response to these items of noncompliance (one Severity Level V and two Severity Level VI. 'Supplement II) is presented in the enclosed Attachments A, B, and C.

142 ATTACHMENT A NOTICE OF VIOLATION As a result of the inspection conducted on August 17-21,

1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980),the following violations were identified:

Appendix B of 10 CFR 50, Criterion V, as addressed in paragraph 5.1 of the Quality Assurance Program in Appendix A to the PSAR, states in part:

"Activities affecting quality shall be prescribed by docu-mented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings...."

The Bechtel Quality Assurance Manual ASME Section III, Division I, 1977 Edition, Section 5000, Paragraph 5148, states in part:

"Requirements'or receipt, inspection, storage...are in WFMC-l.

As a minimum WFMC-1 requires that....filler material be kept....

from....unwanted moisture."

Contrary to the above, it was noted on August 20, 1981, that in Unit 2 Rod Room No. 4, unwanted moisture (lunch) was found in one rod oven containing low hydrogen electrodes.

'Corrective Ste s Taken and Results Achieved The lunch found in the rod oven in Unit 2 Rod Room No.

4 was immediately removed.

The rod room attendant was termimated for this violation of Bechtel procedures and policy.

Corrective Ste s That Have Been Taken to Avoid Further A memo was issued to all rod room attendants on August 21, 1981, which restated Bechtel procedures and jobsite rules which prohibit unauthorized use of rod ovens.

Unannounced inspections of rod rooms. are conducted on a continuing basis',

these inspections have not revealed further violations.

Docket No. 50-528 and 50-529 ATTACHMENT B Construction Permit Nos.

142 and 143 NOTICE OF VIOLATION 2.

Bechtel WPP/QCI 13.0, "Housekeeping",

requires that access control be maintained in zone III designated areas.

Contrary to the above, uncontrolled access was observed on August 17, 1981, in the Unit 1 zone III designated fuel pool area from the fuel transfer tube area into the fuel pool; and on August 20, 1981, in the Unit 1 zone III designated fuel pool area through an open door above the fuel pool.

This is a Severity Level VI Violation (Supplement II) and is applicable to Docket Nos.

50-528 and 50-529.

Corrective Ste s Taken and Results Achieved Work in progress allowed access to the fuel pool that would not normally have been available.

Potential access points, other than through the guard, have been eliminated.

Corrective Ste s That Have Been Taken to Avoid Further Noncom liance A Procedure Change Notice for WPP/QCI 13.0 has been written to more clearly define the control and imple-mentation requirements of cleanliness zones.

Guards have been briefed on the proper completion of control log forms, and access points have been re-arranged to provide the guard a better area overview and control.

Subsequent surveillances have verified compliance.

141 ATTACHMENT C NOTICE OF VIOLATION 3.

Bechtel WPP/QCI 58.0, "Erection of Structural and Miscellaneous Steel", section 4.4.4 states:

"Where long slotted holes are used on an outer ply, a plate washer or continuous bar of at least 5/16 inch thickness with standard holes shall be provided.

These washers or bars shall have a size 'sufficient to completely cover the slot after installation."

Contrary to the above, it was noted on August 19, 1981, that a Unit 2 Containment structural steel platform at elevation 149'ncluded three bolted joints with oversized slots not covered with a plate washer or continuous bar.

Inspection documentation indicated that the platform had been inspected by 'QC in April,

1981, and was considered satisfactory.

This is a Severity Level V Violation (Supplement II) and is applicable to Docket No. 50-529.

Discrepancies found have been documented on Nonconformance Reports and corrected.

Corrective Ste s That Have Been Taken to Avoid Further Noncom liance An inspection was conducted of: the remaining platforms installed in the Unit 2 Containment Building; no addi-tional discrepancies were identified.

A training session was conducted for all QCE personnel to reiterate the requirements of AISC Specifications and WPP/QCI 58.0.
