The following clarification was'nadvex'tently left out of the reference letter resolving human factors concerns:

9-6:
The incoming voltmeter is used along with the synchroscope fox synchronization at the diesel onto the 4.16kv bus.
These meters are about 2 to 3 feet from the breaker controls.
A
'alue is not read from these indicators; the operator is using them to see that the voltages are approximately equal.
Once done the operator uses the synchroscope to determine when to close the breaker.
The 'synchroscope was identified by the control room study as being too far away and will be moved closer to the breaker controls.
