Dear Sir:

The NRC was notified of a potentially reportable deficiency in the referenced telephone conversation. At that time, it was estimated that a determination of reportability would be made within thirty (30) days.

INTERIM REPORT POTENTIAL REPORTABLE DEFICIENCY ARIZONA PUBLIC SERVICE COMPANY (APS)

PVNGS'UNIT il2 I. Potential Problem A concrete void in Unit 2 Control Building sump exists in the basemat adjacent to the West wall of the sump. The void re-.

tains water when the sump is filled and releases water when the sump is emptied; however, the size of the void has not been determined. The cause is attributed to lack of complete concrete consolidation due to reinforcement steel. density.

This condition was not found and corrected during inspection and acceptance of the concrete placement because no surface indication was apparent.

II. A roach'To and Status Of Pro osed Resolution The condition is to be repaired by chipping existing concrete from the area of the void to facilitate inspection and to determine the type and extent of concrete repair required.

Once the size and extent of the void is determined, a decision on its safety significance can be made.

III. Pro'ected Com letion of Corrective'Action and Submittal of Date of 'the"'Final Re 'ort The evaluation and necessary repair are forecast to be completed by March 1, 1981.

The final report will be submitted by March 15, 1981.
