4 The NRC was notified of a potentially reportable deficiency in the referenced telephone conversation. Reference (2) transmitted an Interim Report which estimated completion of the investigation on November 21, 1980.

Due to the extensive investigation and evaluation required, a second Interim Report is attached. It is now expected that this information will be finalized by March 1, 1981, at which time a complete report will be submitted.

INTERIM REPORT POTENTXAL REPORTABLE DEFICIENCY ARIZONA PUBLIC SERVICE COMPANY (APS)

PVNGS UNITS //1, 82 AND //3 I. Potential Problem After Bechtel's receipt of the Callaway/Wolf Creek 10CFR 50.55(e) report concerning Corner & Lada (C&L) Sway Struts, dated July 24, 1980, an inspection was initiated to review the Sizes 1, 2 and 3 sway struts delivered to the PVNGS jobsite. The following conditions have been identified as potential deficiencies:

1. Loose bushings in Size 2 and 3 sway struts at the pipe clamp end can result in the paddle becoming completely disengaged from the bushing, causing a 3/8" gap between paddle and pin.
2. Existence, of a 3/8" gap between the paddle and pin during a seismic event, may result in piping and/or piping welds being over-.

stressed due to movement of the piping system.

After receipt of Bechtel's Quality Assurance Bulletin 80-30 and Problem Alert TPM-P-80-6, October 13, 1980, concerning a generic industry-wide concern for these products and instal-lations, this project has expanded the scope of this potential problem to include the installation of all C&L sway struts.

'Additional surveillance activity performed on November 5, 1980 revealed one C&L sway strut .(2-SG-188-H-001), with bushings on both ends disengaging. The bushings were not staked.

II. A 'roach To and Status of Pro osed Resolution

1. Bechtel Engineering will obtain a written statement from C&L as to the implemented effective date of a formal staking program.
2. Bechtel Construction will identify all struts which have been received prior to the formal staking by C&L.
3. C&L will develop and submit for Bechtel Engineerimg approval, an inspection and repair procedure.
4. Bechtel Construction and C&L will establish how the inspection and repair procedure will be implemented.

5. Defective sway struts will be repaired and/or replaced as established by the approved pro-cedure.

III. Pro'ected Com letion of Corrective Action and Submittal Date of the Final Re ort It is forecast to have the resolution and completion plan implemented by February 1, 1981.

The final report will be submitted by March 1, 1981.
