Dear Sir:

The NRC was notified of a potentially reportable deficiency in the referenced telephone conversation. At that time, it was estimated that a determination of reportability would be made within thirty (30) days.

Due to the extensive investigation and evaluation required, an interim report is attached. It is now expected that 'this information will be finalized by March,1, 1981, at which time a complete report will be submitted.

INTERIM REPORT POTENTIAL REPORTABLE DEFICIENCY ARIZONA PUBLIC SERVICE COMPANY (APS)

PVNGS UNITS 81 AND."82" I. Potential Problem Project receipt of Bechtel's Quality Assurance Bulletin 80-30 and Problem Alert TPM-P-80-6, October 13, 1980, indicated that ITT-Grinnell manufactured sway struts, snubbers and shock sup-pressors have been found to containartially or totally dis-engaged self-aligning rod end bushings.

The condition of total disengagement may create a safety hazard of overstress in piping systems.

Surveillance activity was initiated to evaluate the ITT-Grinnell product received at the jobsite. The results of the review re-vealed that two sway struts (3-ED-401-H-004) and (3-ED-176-H-013) were found with bushings disengaging. The bushings were not staked.

II. A roach To and Status Of Pro osed Resolution

1. Bechtel Engineering will obtain a written statement from ITT-Grinnell as to the implemented effective date of a formal staking program.

2. Bechtel Construction will identify all struts which have been received prior to the formal staking by ITT-Grinnell.

3. ITT-Grinnell will develop and submit for Bechtel Engi-neering approval, an inspection and repair procedure.

4. Bechtel Construction and ITT-Grinnell to establish how the inspection and repair procedure will be implemented.

5. Repair and/or replace defected sway struts as established

~4 by the approved procedure.

III. Pro ected Com letion of Corrective Action and Submittal Date of the Final Re ort It is forecast to have the resolution and completion plan implemented by February 1, 1981.

The final report will be submitted by March 1, 1981.
