Descri tion of Deficienc The project was notified by Comsip Delphi, Incorporated that six hydrogen monitors (two per Unit Tag Numbers 1JHPA-E02, 1JHPB-E02, 2JHPA-E02, 2JHPB-E02, 3JHPA-E02 and 3JHPB-E02) delivered to the jobsite may have a defect which could render the equipment inoperative. The possible defect exists in the motor pump combination of the hydrogen monitor wherein a motor shaft broke after-four hours of extreme test operations. During this test, the panel outlet valves were shut and the pump relief valves were set at about 100 psig; normally, the pumps are subject to no more than 85 psig.

As indicated by the attached letter, Comsip, Incorporated to Bechtel Power Corporation, dated August 6, 1980, Comsip has submitted a report on this condition per the require-ments of 10CFR Part 21.

II. Anal sis of Safet Im lications It cannot be determined without extensive analysis whether the above condition; if not detected, could have resulted in a safety significant event since the PVNGS operating conditions may never be as severe as that of the test.

However, this condition is considered reportable based on the supplier's recommendation to incorporate the redesigned shaft into the delivered equipment to increase the safety

'factor, and to allow for the eventuality of severe operating conditions.

III.'orrective Action The project is, taking the necessary actions to have the subject equipment reworked per the supplier's recommendation. The rework will be accomplished following established field change procedures prior to system start-up. In accordance with NCR J-X-131, rework is to be performed per Design Change Packages (DCP<'s) 1-CJ-HP-001, 2-CJ-HP-001 and 3-CJ-HP-001.

The purpose of this letter is to notify you that Comsip, Inc.,

Delphi Systems Division, has filed a report for the possible existence of a reportable defect, as per the requirements of Code 10, Federal Regulations, Part 21.

See the attached letter dated July 31, 1980.

The possible defect exists in the motor-pump combination of the referenced equipment. ""The problem surfaced on the 29th of July, 1980. Four motor-pump units were being operated at extreme test conditions. The systems were being operated with the panel outlet valves shut and the pump relief valves set at at least 100 psig. One motor shaft broke after four hours of these extreme operating conditions.

Normally, with the pump relief valves set properly and the system outlet valve shut,'he'umps see no more than 8S psig.

In any case, in this condition the low sample flow alarms will sound and normally within an hour an operator will shut the motor off.

As indicated in the attached report, the motor shaft has been redesigned to increase the safety factor. Even though the failure occurred under extreme conditions, it is desirable to update the equipment to allow for such eventualities.

1 August 6, 1980 Page 2 Comsip will provide motor-pump units with improved safety factor with installation'nstructions and fittings to all customers at a cost of $ 3,640.00. However, Comsip will refund $ 2,890.00 for every motor pump combination returned to our factory.

These units will be available October 1980.

If required, a Comsip Field Service person will be avail-able for supervising installation of these units. The rate for this service is $ 425.00 per day plus living and travel expenses.
