PVNGS UNIT //2 I.
Descri tion of Deficienc Forty-four (44) No.
6 dowels were inadvertently omitted from the top of a Control Building wall at Elevation 180'Wall Pour //2J054).
The nonconforming condition of the wall was discovered during the preplacement QC inspection for the roof pour.
This condition is attributed to oversights by the personnel and inspectors when placing the rebar in the wall prior to the wall placement.
II.
Anal sis of Safet Im lications This condition is considered to be reportable based on these considerations:
- l. If not detected and corrected as indicated, the wall/roof connection could possibly ex-perience local structure failure under seismic conditions.
2.
This condition required an extensive evaluation and repair procedure.
3.
This condition was not detected by the normal inspection process.
III. Corrective Action The condition is being corrected following the approved NCR (No. C-J-2076) disposition to drill and grout the required dowels into the wall following the design drawing requirement.
Since DER 80-13 documented a similar situation (40 missing dowels in Unit //3 Auxiliary Building at Eleva-tion 70'), corrective action for this Unit //2 deficiency was transferred to CAR S-80-95.
DER 80-13 was reported to the NRC on June 5, 1980 with a final report submitted on July 8, 1980 (Letter ANPP-15853).
In order to prevent recurrence, on September.
10, 1980 at 1:00 p.m.,
CAR S-80-95 was discussed at the General Super-intendents'eeting by the Project Superintendent.
Quality Control'personnel were also in attendance.
Emphasis was placed on correct reading of drawings and prevention of oversights in the installation of dowels.
