I FINAL REPORT REPORTABLE DEFICIENCY 50.55(e)

ARIZONA PUBLIC SERVICE COMPANY (APS)

PVNGS UNITS 81, 'f/2 AND f/3 I Descri tion of Deficienc Borg-Warner, a subsupplier to Combustion Engineering for the NSSS, found, during a test of 1", 1<" and 3" valves, that the stem extension used for position indication could loosen after a few cycles. If the stem extension came off, it could prevent valve movement.

Bechtel has verified, by telecon with Borg-Warner, that a 10CFR Part 21 report has been filed with the NRC to cover the parts supplied to PVNGS as part of the Combustion Engineering NSSS contract.

II Anal sis of Safet Im lications This condition is considered reportable, since it acknow-ledges the deficiency described in the 10CFR Part 21 report filed by Borg-Warner.

III Corrective Action Bechtel has tagged the identified valves and initiated NCR-PX-1412 to cover the valve modification rework. Borg-Warner will supply Combustion Engineering with instructions and parts for the valves, in turn, Combustion Engineering will coordinate the modification implementation with Bechtel construction personnel at the jobsite. Completion of this modification prior to system turn-over for startup will be accomplished following established NCR procedures.
