SUBJECT: NRC SPECIAL INSPECTION OF WASHINGTON NUCLEAR PROJECT-2 (NRC
INSPECTION REPORT 50-397/98-05) AND NOTICE OF VIOLATION
Dear Mr. Parrish:
From March 12-17, 1998, a special inspection was'conducted
at your Washington Nuclear
Project-2 reactor facility. The team examined the facts surrounding the March 11, 1998, main
steam isolation valve closure and reactor scram.
On April 8 an exit meeting, open to public
observation, was held to discuss the results of this inspection with you and other members of
your staff.
The charter for this special inspection is attached to the enclosed inspection report. The findings
and conclusions of the special inspection team are documented
in the. report.
Four violations were identified during this special inspection.
The violations involved the failure
to report a valid emergency core system injection within one hour, a failure to maintain the
indicated reactor vessel temperature and pressure within the limits established
by the cooldown
surveillance procedure, the use of a temporary change notice to alter the intent of a logic system
functional procedure, and inadequate procedures for the testing and restoration of safety-related
equipment.
These violations are of a concern because of the common management oversight
(command and control) weaknesses
that contributed to each violation. The violations are cited
in the enclosed Notice of Violation (Notice) and the circumstances
surrounding the violations are
described in detail in the enclosed report.
Please note that you are required to respond to this
letter and should followthe instructions specified in the enclosed Notice when preparing your
response..The
NRC willuse your response,
in part, to determine whether further enforcement
action is necessary to ensure compliance with regulatory requirements.
An unresolved item was identified regarding implementation of the maintenance rule for the
reactor core isolation cooling system.
The issue involves whether adequate preventive
maintenance had been implemented to demonstrate that the performance of the risk significant
reactor core isolation cooling system was being effectively controlled through the performance of
appropriate preventive maintenance on the test return line throttle and isolation valves in
accordance with the requirements of 10 CFR 50.65(a)(2).
The valves were elected not to be
placed in the Generic Letter 89-10, "Motor-Operated Valve Testing and Surveillance," program
and your staff did not believe that these valves should be included within the maintenance
rule.

This issue will be reviewed further by NRC to determine ifyour staff's position is appropriate and
meets regulatory requirements.
The NRC is concerned with the poor communications that occurred with the NRC and in,
particular the NRC operations officer following the main steam isolation valve closure and
reactor scram.
The initial report and subsequent
update, which also reported the second reactor
protection system actuation, did not adequately address the plant conditions or demonstrate that
the control room personnel had an appropriate awareness
of the plant status.
The NRC relies
extensively on the accuracy of initial event reports and any updates in ascertaining whether the
plant event was understood and whether immediate NRC followup actions should be taken.
This poor communication contributed significantly to the NRC decision to initiate this special
team inspection to assess
the plant event and verify that appropriate followup actions had been
and were being taken.
We are also concerned with the type and number of operator performance issues that were
identified by your staff and the team. The NRC is concerned with the operators'erformance,
including management
oversight, and recognition of expected safety system performance.
Based on the findings from this special inspection, a management
meeting, open to public
observation, will be held in the Region IV office on April 30, 1998. This meeting will be
integrated into the oversight meeting currently scheduled for the same day. The discussion
should include the insights that were gained from your staffs review of this event and any
related findings from your self assessment.
Specific areas for discussion should include
operator performance, the adequacy of intra and inter communications, and the effectiveness of
the corrective action process to identify and correct operator and equipment performance
issues.
In accordance with 10 CFR 2.790 of the NRC's "Rules of Practice," a copy of this letter, its
enclosure(s),
and your response will be placed in the NRC Public Document Room (PDR).

with you.
