NRC INSPECTION REPORT 50-397/97-19 AND NOTICE OF VIOLATION
Dear Mr. Parrish:
An NRC inspection was conducted November 17-21, 1997, at your Washington Nuclear
Project-2 reactor facility. Additional information was provided on December 5, 1997, during a
telephone conversation between Mr. L. Ricketson of Region IVand Mr. D. Hillyerand other
members of your staff. The enclosed report presents the scope and results of that inspection.
Two violations were identified during the review of selected radiation protection activities. One
violation involved the failure to control personnel access to an area in which radiation dose rates
exceeded
1 rem per hour. The second violation involved the failure to evaluate radiological
hazards resulting from the potential intake of radioactive material. Additionally, numerous
procedural guidance weaknesses
were noted.
These violations are cited in the enclosed Notice of Violation (Notice) and the circumstances
surrounding the violations are described in detail in the enclosed report.
Please note that you
are required to respond to this letter and should followthe instructions specified in the enclosed
Notice when preparing your response.
The NRC willuse your response,
in part, to determine
whether further enforcement action is necessary
to ensure compliance with regulatory
requirements.
In accordance with 10 CFR 2.?90 of the NRC's "Rules of Practice," a copy of this letter, its
enclosure(s),
and your response will be placed in the NRC Public Document Room (PDR). To
the extent possible, your response should not include any personal privacy, proprietary, or
