SUBJECT:
NRC INSPECTION REPORT 50-397/97-03 AND NOTICE OF VIOLATION
Dear Mr. Parrish:
An NRC inspection'as
conducted January'19 through March 1, 1997, at your
Washington Nuclear Project-2. rea'ctor facility. The enclosed report presents the'cope
and
results of that inspection.
During the 6-week period covered by this inspection, your conduct of activities at the
Washington Nuclear Project-2 facility was generally characterized
by a significant increase
in the human performance
error rate.
We did note, however, that your Operations staff
responded'ell to a reactor recirculation control pump trip.
Three violations of NRC requirements
were identified during this inspection.
The first two
violations address
a number of events that occurred as a result of human performance
errors in the areas of.Operations
and Maintenance.
In several of these instances your staff
demonstrated'ailures
of self and independent
verification.
The third violation involv'es your
use of chemistry technicians
as o'-shift health physics technicians to meet the staffing
requirements of your Emergency Plan.
This violation is of concern because
your actions
~
were not consistent with assuring qualification.requirements
were maintained and when
this issue was identified on February 10, 1997, corrective actions were not taken until four
'ays later.. Furthermore,
a previous quality assurance
finding in this area had not been
appropriately resolved.
In addition to the three violations, we are concerned with two additional 'events that. were
a so caused
by, human performance problems.
In one event, a reactor recirculation pump
I
tripped when a contractor used
ari, inappropriate
probe and shorted
a circuit to ground.
~ The individual involved with the e'vent and the engineer providing contractor oversight
were both aware of a standing order which prohibited the contractor involved from
performing. hands-on work at WNP-2. The second event. of concern involved the failure of
a rod block monitor when an uninsulated screwdriver was accidently dropped and shorted
the rod block monitor power supply.
The technician who performed the work was aware
of management's
expectations
regarding the use of insulated screwdrivers.
These issues
are of concern because
they involve instances
where w'orkers did not meet management
expectations.

We have noted increases
in the human performance
error rate near the first of the year has
been
a historical problem and that you had taken some actions prior to this period and also
additional actions after the human performance
events.
We encourage
your continued
management
attention to this area.
The violations are cited in the enclosed
Notice of Violation (Notice) and the circumstances
surrounding the violations are described
in detail in the enclosed report.
Please note that
you are required to respond to this letter and should follow the instructions specified in the
enclosed
Notice when preparing your response.
The NRC will use your response,
in part,
to determine whether further enforcement action is necessary
to ensure compliance with
regulatory requirements.
In accordance
with 10 CFR 2.790 of the NRC's "Rules o'f Practice,"
a copy of this letter,
its enclosures,
and your response
will be placed io the NRC Public Document Room. (PDR).
To the extent possible, your response
should not include any personal
privacy', proprietary,
or safeguards
information so that it can be placed in the PDR without redaction.
