NOTICE OF VIOLATION Washington Public Power Supp'ly System Washington Nuclear Project-2 Docket:

50-397 License:

NPF-21 During an NRC inspection conducted on March 31 through May 11,

1996, one violation of NRC requirements were identified.

In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions,"

NUREG - 1600, the violation is listed below:

10 CFR Part 50, Appendix 8, Criterion XVI, requires, in part, that measures be established to assure that conditions adverse to quality, such as deficiencies and nonconformances, are promptly identified and corrected.

In the case of significant conditions adverse to quality, the measures shall ensure that the cause of the condition is determined and corrective action taken to preclude repetition.

Contrary to the above, licensee corrective actions in two instances were not effective in precluding the repetition of significant conditions adverse to quality.

As of April 7, 1996, corrective actions from a previous violation.

This was a similar problem to one identified in the 1994 refueling outage.

As of August 22, 1995, corrective actions from a previous licensee event report associated with exceeding heat exchanger fouling limits for the standby service water system room cooler were not effective at precluding repetition in that cooler efficiencies decreased below the established limits for a second time.

This is a Severity Level IY violation (Supplement I).

The NRC has concluded that information regarding the reason for this violation, the corrective actions taken and planned to correct the violation and prevent recurrence, and the date when full compliance will be achieved is already adequately addressed on the docket in Inspection Report 397-50/96-06, attached.

However, you are required to submit a written statement or explanation pursuant to 10 CFR 2.201 if the description therein does not accurately reflect your corrective actions or your position.

In that case, or if you choose to respond, clearly mark your response as a "Reply to a Notice of Violation," and send it to the U.S. Nuclear Regulatory Commission, ATTN:

20555 with a copy to the Regional Administrator, Region IV, and a copy to the NRC Resident Inspector at the facility that is the subject of this Notice, within 30 days of the date of the letter transmitting this Notice of Violation (Notice).

Because the response will be placed in the NRC Public Document Room (PDR), to the extent possible, it should not include any personal privacy, proprietary, or safeguards information so that it can be placed in the PDR without redaction.
