REPORT 50-397/96-06
Dear Mr. Parrish:
On May 11,
1996,
the
NRC completed
an inspection
at your Washington Nuclear
Project-2 facility.
The enclosed
report presents
the results of that
inspection.
During the 6-week period covered
by this inspection period, your conduct of
activities at the Washington Nuclear Project-2 facility was generally
characterized
by effective
command
and control
by operations,
sound
engineering
and maintenance
practices,
and careful radiological
work controls.
We are concerned,
however,
about
one violation with two examples of NRC
requirements
that were identified concerning ineffective corrective actions.
This violation is cited in the enclosed
Notice of Violation, and the
circumstances
surrounding it are described
in detail
in the enclosed
report.
No response
for the violation is required.
The violation involved two instances
where planned corrective actions
from a
previous violation and licensee
event report were not effectively implemented,
resulting
in repeat
occurrences
of the initial problems.
In the first
instance,
you failed to properly verify that all the moisture separator
shroud
hold down bolts were
removed before attempting
to lift the shroud.
In the
second
instance,
you failed to adequately
monitor standby service water room
cooler fouling and allowed degradation
below established
limits.
We also
noted
two further examples of a violation cited in
NRC Inspection
Report 50-
397/96-02 involving weaknesses
in instructions
and procedures
for work on the
diesel
generator
heating, ventilation,
and air conditioning dampers.
We note
that these further examples
were identified at the
same
time as the violation
in
NRC Inspection
Report 50-397/96-02
and that you have initiated actions to
address
these
examples.
Nevertheless,
these
examples
demonstrate
the
need for
your continued attention with the control of maintenance activities.
