INSPECTION
REPORT 50-397/95-20
This refers to the inspection
conducted
by Mr. R.
C. Barr,
and the inspectors
identified in this report, of this office on June
4 through July 15,
1995.
The inspection
included
a review of activities authorized for your Washington
Nuclear Project
2 facility.
At the conclusion of the inspection,
the findings
were discussed
with those
members of your staff identified in the enclosed
report.
Areas
examined during this inspection
are described
in the enclosed
inspection
report.
Within these
areas,
the inspection consisted of selective
examinations
of procedures
and representative
records,
interviews with
personnel,
and observations
by the inspectors.
The purpose of the inspection
was to determine
whether activities authorized
by the license
were conducted
safely
and in accordance
with NRC requirements.
We were encouraged
by the well controlled startup
and
power ascension
following Refueling Outage
R10.
However,
the
WNP-2 staff continues
to make
a
number of human
performance errors.
During this inspection period,
these
errors resulted
in
a reactor water level transient,
a control
rod out of its
desired position,
two clearance
order errors
(breakers
out of position),
and
the inadequate
verification of the control
rod drive housing support
installation.
Your continued vigilance in eliminating
human performance
errors is warranted.
Based
on the results of this inspection,
certain of your activities
appeared
to be in violation of NRC requirements,
as specified in the enclosed
Notice of
Violation (Notice).
The first two violations are of concern
because
they
resulted
in an unanticipated
reactor water level transient
and
a failure to
provide
an adequate
procedure for verification of the control
rod drive
housing
support installation.
The third violation,
a failure to perform
a
10 CFR 50.59 evaluation for a temporary modification, is of concern
because
of the inadequate
process
used to determine
whether to perform the safety
evaluation.
You are required to respond
to this letter
and should follow the instructions
specified
in the enclosed
Notice when preparing your response.
In your
response,
you should
document
the specific actions
taken
and
any additional
actions
you plan to prevent recurrence.
In your response
to these violations
please
address
the apparent
differences
between
our inspectors'indings
and
your followup of this issue.
Your response
may reference
or include previous

correspondence, if the correspondence
adequately
addresses
the
required
response.
After reviewing your response
to this Notice, including
your proposed
corrective actions
and the results of future inspections,
the
NRC will determine
whether further
NRC enforcement
action is necessary
to
ensure
compliance with NRC regulatory requirements.
In accordance
with 10 CFR 2.790 of the NRC's "Rules of Practice,"
a copy of
this letter and its enclosures will be placed in the
NRC Public Document
Room
(PDR).
To the extent possible,
your response
should not include any
personal
privacy, proprietary,
or safeguards
information so that it can
be
placed
in the
POR without redaction.
However, if you find it necessary
to
include
such information, you should clearly indicate the specific information
that you desire
not to be placed in the
POR,
and provide the legal basis to
support your request for withholding the information from the public.
The responses
directed
by this letter
and the enclosed
Notice are not subject
to the clearance
procedures
of the Office of Management
and Budget
as required
by the Paperwork
Reduction Act of 1980,
