REPORT 50-397/95-15
This refers to the inspection
conducted
by Nr.
R.
C. Barr,
and the inspectors
identified in this report, of this office on April 23 through June 3,
1995.
The inspection
included
a review of activities authorized for your Washington
Nuclear Project
2 facility.
At the conclusion of the inspection,
the findings
were discussed
with those
members of your staff identified in the enclosed
report.
Areas
examined during this inspection
are described
in the enclosed
inspection
report.
Within these
areas,
the inspection consisted
of selective
examina-
tions of procedures
and representative
records,
interviews with personnel,
and
observations
by the inspectors.
Based
on the results of this inspection,
certain of your activities appeared
to be in violation of NRC requirements,
as specified in the enclosed
Notice of
Violation (Notice).
The violation is of concern
because it resulted
in
personnel
not adhering to requirements
to prevent fires Following grinding
operations.
NRC inspectors
continue to identify a large
number of operator
performance
issues
that principally stem from inattention to detail.
Clearance
order
and
tagging errors continue to persist.
Turnovers
and log keeping
are not of a
consistent
high quality, leading to weak performance
by operators.
Your
continued attention to these
issues
is warranted..
Our observation of the
damage to the high pressure
core spray diesel
generator
regulator indicated
inadequate
coordination
between
operations
and technical
personnel
and that system
knowledge
was weak.
In addition,
operators'esponse
to the incident did not appear to meet your expectations.
You are required to respond to this letter
and should follow the instructions
specified
in the enclosed
Notice when preparing your response.
In your
response,
you should
document
the specific actions
taken
and
any additional
actions
you plan to prevent recurrence.
In your response
to this violation,
please
address
the apparent
differences
between
our inspectors'indings
and
your followup of this issue.
Your response
may reference
or include previous
docketed
correspondence, if the correspondence
adequately
addresses
the
required
response.
After reviewing your response
to this Notice, including
your proposed
corrective actions
and the results of future inspections,

NRC will determine
whether further
NRC enforcement
action is necessary
to
ensure
compliance with NRC regulatory requirements.
In accordance
with 10 CFR 2.790(a) of the
NRC's "Rules of Practice,"
a copy of
this letter
and its enclosures will be placed
in the
NRC Public Document
Room (PDR).
To the extent possible,
your response
should not include
any
personal
privacy, proprietary,
or safeguards
information so that it can
be
placed in the
PDR without redaction.
However, if you find it necessary
to
include such information, you should clearly indicate the specific information
that you desire
not to be placed in the
PDR and provide the legal basis to
support your request for withholding the information from the public.
The responses
directed
by this letter
and the enclosed
Notice are not subject
to the clearance
procedures
of the Office of Management
and Budget
as required
by the Paperwork
Reduction Act of 1980,
