REPORT 50-397/95-14
This refers to the inspection
conducted
by Messrs,
L.
E. Ellershaw,
C. J.
Paulk,
and J.
E. Whittemore of this office on April 24 through
May 23,
1995.
The inspection
included
a review of activities authorized for
your Washington
Nuclear Project-2 facility.
At the conclusion of the
inspection,
the findings were discussed
with those
members of your staff
identified in the enclosed
report.
Areas
examined during the inspection
are identified in the report.
Within
these
areas,
the inspection consisted
of selective
examinations of procedures
and representative
records,
interviews with personnel,
and observation of
activities in progress.
The purpose of the inspection
was to determine
whether activities authorized
by the license
were conducted
safely
and in
accordance
with NRC requirements.
The inspectors
identified two violations of Criterion
V of Appendix
B to
10
CFR Part 50:
a failure to adequately
implement
a procedure for certifying the
visual acuity of nondestructive
test personnel,
and
a failure to have
an
ultrasonic testing procedure
adequately
address criteria for determining that
important activities
had
been
accomplished.
These violations are not being
cited because
the criteria in paragraph VII.B.(l) of Appendix
C to
10 CFR Part 2 of the
NRC's "Rules of Practice,"
were satisfied.
These
were isolated
occurrences
and your staff took prompt
and thorough corrective actions.
Additionally, your personnel
identified the securing of the residual
heat
removal
system while in the shutdown cooling mode for greater
than
3 hours3.472222e-5 days <br />8.333333e-4 hours <br />4.960317e-6 weeks <br />1.1415e-6 months <br /> in
an 8-hour period,
and this was
a violation of the Technical Specifications.
This violation is not being cited because
the criteria in paragraph VII.B.(2)
of Appendix
C to
10 CFR Part 2 were satisfied.
The amount of decay heat
present
at the time of the event
was minimal
and the reactor coolant
temperature
did not rise
a significant amount.
For the specific areas
examined
and the associated
inspection findings, the
inspectors
concluded there
was
an
improvement in licensee
performance of
refueling outage activities
as
compared to that of the previous outage.
In accordance
with 10 CFR 2.790 of the NRC's "Rules of Practice,"
a copy of
this letter
and its enclosure will be placed in the
NRC Public Document
Room.
