AND NOTICE OF VIOLATION

This refers to the inspection

conducted

by Nr.

R.

C. Barr,

and the inspectors

identified in this report, of this office on December ll, 1994,

through

January

21,

1995.

The inspection

included

a review of activities authorized

for your Washington

Nuclear Project-2 facility.

At the conclusion of the

inspection,

the findings were discussed

with those

members of your staff

identified in the enclosed

report.

Areas

examined during the inspection

are identified in the report.

Within

these

areas,

the inspection

consisted

of selective

examinations

of procedures

and representative

records,

interviews with personnel,

and observation of

activities in progress.

The purpose of the inspection

was to determine

whether activities authorized

by the license

were conducted

safely

and in

accordance

with

NRC requirements.

There were

a number of findings b'ased

on the results of this inspection

and

certain licensed activities

appeared

to be in violation of NRC requirements,

as specified in the enclosed

Notice of Violation (Notice).

The findings of

this inspection indicate that certain of your control processes

are not

functioning in

a consistent,

effective manner.

Of particular concern

was our

finding that the shift manager

consulted

only with the licensing

manager

regarding

the operability determination

on the valve position indication for a

postaccident

sampling

system containment isolation valve.

This is

a function

normally provided

by operations

management

and system engineers,

but may

include the licensing manager.

This raises

questions

regarding

operators'ecognition

of the respective

roles of operations

management,

system

engineers,

and the licensing

manager

in making operability determinations

at

WNP-2.

In addition,

we are concerned

that your operators

do not consistently,

rigorously evaluate

degraded

equipment to assure

conformance with the

Technical Specifications,

as occurred with the postaccident

sampling

system

valve.

Other findings indicated that your expectations

for, self-checking

and

second

or independent verification have not been consistently

implemented.

On

multiple occasions,

an operator

and

a second

individual failed to verify that

control rod manipulations

had

been properly performed.

Several

additional

examples of weaknesses

in second

or independent verification have

been

identified since this inspection.

In addition, your operators

have not

demonstrated

appropriate attention to detail.

For instance,

operators

have failed to assure

that the diesel

generator

governor

speed

switch setting

was proper.

Additionally, you identified 'that your craftsmen

performed

a modification to

a

replacement

scram discharge

volume level switch without

a proper evaluation or

approved

work package

(a noncited violation) and that your procurement

and

receipt inspection

programs failed to assure

that

a fully adequate

scram

discharge

volume level switch was procured or verified.

Your actions to

correct these

problems

were timely and appropriate.

We urge your contioued

attention to assuring that these

important activities are conducted

in

a fully

satisfactory

manner.

In accordance

with Section VII.B.(2) of 10 CFR Part 2,

Appendix C,"General

Statement

of Policy and Procedure for NRC Enforcement

Actions," this violation,was not cited.

In evaluating

the Supply System's

performance of maintenance

during reactor

operation

(online maintenance),

the inspectors

found that you thoroughly

consider

the safety (risk) aspects

of conducting that maintenance.

Your risk

assessments

rely heavily on the experience of the Production Scheduling Shift

Hanager

(PSSH).

We understand

your future plans include providing the

PSSH

with additional

resources,

such

as

a system dependency

matrix, to assist

the

PSSH in performing his assessments.

You are required to respond

to this letter

and should follow the instructions

specified in the enclosed

Notice when preparing your response.

In your

response,

you should

document

the specific actions

taken

and

any additional

actions

you plan to prevent recurrence.

Your response

may reference

or

include previous docketed

correspondence, if the correspondence

adequately

addresses

the required response.

After reviewing your response

to this.

Notice, including your proposed

corrective actions

and the results of future

inspections,

the

NRC will determine

whether further

NRC enforcement

action is

necessary

to ensure

compliance with NRC regulatory requirements.

In accordance

with 10 CFR 2.790 of the NRC's "Rules of Practice,"

a copy of

this letter, its enclosure(s),

and your response will be placed in the

NRC

Public Document

Room (PDR).

To the extent possible,

your response

should not

include

any personal

privacy, proprietary,

or safeguards

information so that

it can

be placed in the

PDR without redaction.

However, if you find it

necessary

to include

such information, you should clearly indicate the

specific information that you desire

not to be placed in the

PDR and provide

the legal basis to support your request for withholding the information from

the public.
