This refers to the inspection
conducted
by
identified in the enclosed report, of this
April 2,
1994.
This inspection
included
a
your Washington Nuclear Project 2.
At the
findings were discussed
with you and those
the enclosed report.
Mr. R.
C. Barr,
and the inspectors
office on February
22 through
review of activities authorized for
conclusion of the inspection,
the
members of your staff identified in
Areas
examined during this inspection
are described
in the enclosed
inspection
report.
Within these
areas,
the inspection consisted of selective
examina-
tions of procedures
and representative
records,
interviews with personnel,
and
observations
of activities in progress.
Several
issues
contained
in this report highlight that improved engineering
support of plant operations
is warranted.
Specifically,
a number of
deficiencies
were identified by the inspectors
regarding the control
room
heating, ventilation,
and air conditioning system.
These deficiencies
could
have
been identified and corrected
by your engineering staff, or by other
members of the plant staff, if a more questioning attitude existed.
Additionally, engineering
support of supply system considerations
to extend
the present
fuel cycle and to implement
changes
to rated core power during the
upcoming outage,
could have
been
more thorough.
Specifically, senior
management
failed to provide effective and timely communication of their plans
to extend Cycle
9 operation to the fuels engineering organization.
This
resulted
in extensive efforts to provide
a new core reload design analysis
at
a late date in the fuel cycle.
We are concerned that your programs to assure
completion of NRC commitment's
are not effective.
Specifically, in your response
to the
1993
SALP report,
you commited to perform system walkdowns with teams of personnel.
However,
several
previous
NRC inspection reports
have documented that your staff had
not performed
system walkdowns with the necessary
frequency or the requisite
degree of detail to assure
a high quality condition of your safety systems.
The inspectors
found, during this inspection period, that the walkdowns of the
control
room
HVAC system were not of sufficient detail or depth to identify

and correct the types of deficiencies
which were identified by the Resident
Inspectors
during their walkdowns of the system.
Based
on the results of this inspection,
certain of your activities appeared
to be in violation of NRC requirements,
as specified in the enclosed
Notice of
Violation (Notice).
These violations concerned your failure to take prompt
corrective action for the degraded
condition of your control
room emergency
chiller, which is required to be operable
by the Washington Nuclear Project
2
operating license,
and your failure to follow procedures
for control
room
ventilation and to provide appropriate
procedural
direction for breaker
lineups.
You are required to respond to this letter and should follow the instructions
specified in the enclosed
Notice when preparing your response.
In your
response,
you should
document the specific actions
taken
and any additional
actions
you plan to prevent recurrence.
After reviewing your response
to this
Notice, including your proposed corrective actions
and the results of future
inspections,
the
NRC will determine
whether further
NRC enforcement
action is
necessary
to ensure
compliance with NRC regulatory requirements.
In accordance
with 10 CFR 2.790(a) of the NRC's "Rules of Practice,"
a copy of
this letter and the enclosure will be placed in the
NRC Public Document
Room.
