NOTICE OF VIOLATION (NRC

INSPECTION

REPORT

NO. 50-397/94-02)

This refers to a special

team inspection

conducted

by Hr.

P. Narbut

and

a team

of NRC and contractor personnel

from January

10-14

and January

24-28,

1994, of

activities authorized

by

NRC Operating

License

No. NPF-21,

and to the

discussions

of findings held by the inspectors with you and other members of

the Supply System staff at the conclusion of the inspection.

Additionally,

the results of the inspection

conducted

by Hr. D. Kirsch from November

29

through

December

3 and December

13-17,

1993,

are included in the enclosed

inspection report.

The inspection

was

an examination of the effectiveness

of engineering

and

technical

support.

Areas

examined during this inspection

are described

in the

enclosed

inspection report.

Within these

areas,

the inspection consisted

of

selective

examinations of procedures

and representative

records,

interviews

with personnel,

and observations

by the inspectors.

We noted that you have recently

implemented

several

important improvement

initiatives which appear to be having positive results.

Examples of these

initiatives include your engineering

backlog reduction, your temporary

modification reduction,

and the improved interface

between

engineering

and

other organizations.

Although it is too early to assess

the long term

effectiveness

of your engineering initiatives,

we are encouraged

by your

efforts thus far.

The team also noted that your decision to install

new

metallic-seated

containment

purge

and supply isolation valves,

in order to end

a longstanding

problem with leaks in the rubber-seated

valves,

was

commendable.

Although the team found that engineering

support of plant operations

has

generally improved,

a recent

issue deviates

from that conclusion.

Engineering

appears

to not have

been timely in their review of fuel loading options for

cycle 10.

This issue is still under

NRC review and will be addressed

in a

later inspection report.

The team noted two significant unresolved

items

which warrant additional

management

attention.

The unresolved

items involve:

(1) the adequacy of your proposed modification and the

10 CFR 50.59 safety

evaluation for the

BWR level instrumentation backfill system,

and (2) the

Additionally, based

on the results of this inspection, it appears

that

one of

your activities was not conducted

in full compliance with NRC requirements,

as

set forth in the enclosed

Notice of Violation (Notice).

The violation

involved the failure to periodically verify the position of manual

containment

isolation valves in accordance

with technical specification requirements.

The

involved valves were

115 manual

bypass

valves for the containment isolation

excess

flow check valves.

You are required to respond to this Tetter and should. follow the instructions

specified in the enclosed

Notice when preparing your response.

In your

response,

you should document the specific actions taken

and

any additional

actions you plan to prevent recurrence.

After reviewing your response

to this

Notice, including your proposed corrective actions

and the results of future

inspections,

the

NRC will determine

whether further

NRC enforcement

action is

necessary

to ensure

compliance with NRC regulatory requirements.

In accordance

with 10 CFR 2.790(a),

a copy of this letter, the enclosures,

and

your response will be placed in the

NRC Public Document

Room.

The response

directed

by this letter

and the accompanying

Notice are not

subject to the clearance

procedures

of the Office of Hanagement

and Budget

as

required

by the Paperwork Reduction Act of 1980,

Pub.

L.96-511.
