This refers to the inspection
conducted
by Hessrs.
R.
C. Barr,
D. L. Proulx,
S.
P. Sanchez,
and
D.
E. Corporandy of this office on November 30,
1993,
through January
10,
1994, of activities authorized
by
NRC Operating
License
No. NPF-21,
and to the discussions
of findings held by the inspectors with
Hr. J.
W. Swailes
and other members of the Supply System staff at the
conclusion of the inspection.
Areas
examined during this inspection
are described, in the enclosed
inspection
report.
Within these
areas,
the inspection consisted
of selective
examina-
tions of procedures
and representative
records,
interviews with personnel,
and
observations
by the inspectors.
Based
on the results of this inspection, it appears
that certain of your
activities were not conducted
in full compliance with NRC requirements,
as set
forth in the enclosed
Notice of Violation (Notice).
This violation involved
two failures to follow plant procedures
which required the initiation of
problem evaluation
requests
(PERs)
when
a discrepancy
was discovered
upon
closeout of a maintenance
work request
(HWR), and the restraint of compressed
gas cylinders in safety related
areas.
Four non-cited violations were also
noted.
You are required to respond to this letter
and .should follow the instructions
specified in the enclosed
Notice when preparing your response.
In your
response,
you should document the specific actions
taken
and
any additional
actions
you plan to prevent recurrence.
After reviewing your response
to this
Notice, including your proposed corrective actions
and the results of future
inspection,
the
NRC will determine
whether further
NRC enforcement
action is
necessary
to ensure
compliance with NRC regulatory requirements.
In accordance
with 10 CFR 2.790(a),
a copy of this letter, the enclosure,
and
your response will be placed in the
NRC Public Document
Room.
