NOTICE OF VIOLATION Washington Public Power Supply System Docket No. 50-397 Washington Nuclear Project No.
2 License No. NPF-21 During an NRC inspection conducted on June 22 through August 2,
- 1993, a
violation of NRC requirements was identified.
In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions,"
10 CFR Part 2, Appendix C, the violation is listed below:
10 CFR 50, Appendi x B, Criteri on V, requires, in part, that acti vities affecting quality be prescribed by procedures of a type appropriate to the circumstances.
Contrary to the above, the, following two WNP-2 procedures were found to be inappropriate to the circumstances:

As of July 16,
- 1993, WNP-2 Plant Procedures Manual (PPM) 9.3. 1, "Manual Core Heat Balance,"
Revision 6, was inadequate in that it directed data to be taken for total reactor water cleanup (RWCU) flow by using computer data points which did not reflect the actual flow through the bypassed RWCU deminerilizers.
This error allowed a
non-conservative value for the calculation of core thermal power.

As of June 22,
- 1993, PPM 7.4.1.1, "Reactor Shutdown Margin and/or Demonstration,"
was inadequate in that it did not provide direction to the user regarding which value of keff to use to calculate the effective neutron multiplication factor (keff) with all rods in.
PPM 7.4.1. 1 referenced two different values in the "Startup Operations Letter Report."
PPM 7.4. 1. 1 also did not provide direction to the user regarding which of the two referenced values for moderation temperature coefficient (MTC) should be used.
As a
- result, the wrong values for keff and the MTC were used during the performance of PPH 7.4. 1. 1 on June 21, 1993.
This is a Severity Level IV violation (Supplement I).
Pursuant to the provisions of 10 CFR 2.201, Washington Public Power Supply System is hereby required to submit a written statement or explanation to the U. S. Nuclear Regulatory Commission, ATTN:
Document Control Desk, Washington, DC 20555, with a copy to the Regional Administrator, Region V, and a copy to the NRC Resident Inspector at WNP-2, within 30 days of the date of the letter transmitting this Notice of Violation (Notice).
This reply should be clearly marked as a "Reply to a Notice of Violation" and should include for each violation:
(1) the reason for the violation or, if contested, the basis for disputing the violation, (2) the corrective steps that have been taken and the results
- achieved, (3) the corrective steps that will be taken to avoid further violations, and (4)'he date when full compliance will be achieved.
If an adequate reply is not received within the time specified in this Notice, the Commission may issue an order or a request for information as to why the license should not be modified, suspended, or revoked or why such other action as may be proper should not be taken.
Where good cause is shown, consideration will be given to extending the response time.
