NOTICE OF VIOLATION FOR THE SERVICE MATER TEAM INSPECTION
NRC INSPECTION
REPORT
NO. 50-397/93-201
This refers to the findings identified during the team inspecti on
1 ed by
Mr J.
B. Jacobson of the Office of Nuclear Reactor Regulation
{NRR) from
February
1 through
11,
1993.
The'nspection
included
a review of activities
authorized for your WNP-2 facility and focused
on the service water system.
At the conclusion of the inspection,
the findings were discussed
with those
members of your staff identified in the subject r'eport issued April 1,
1993.
As was explained at that meeting,
any enforcement
matters
would be addressed
separately
by Region V.
This letter forwards the violations identified as
a
result of the inspection.
Our conclusions
regarding these violations were
discussed
with Mr. J.
W. Baker and other members of your staff on April 26,
1993.
Areas
examined during the inspection
were identified in the subject report.
The inspection
examined the operational
performance of the service water
system.
Within these
areas,
the inspection consisted of selective
examinations of procedures
and representative
records,
interviews with
personnel,
and observation of activities in progress.
The purpose of the
inspection
was to determine whether activities authorized
by the license
were
conducted
safely
and in accordance
with NRC requirements.
Based
on the results of'he inspection,
certain of your activities appeared
to
be in violation of NRC requirements,
as specified in the enclosed
Notice of
Violation {Notice).
The violations involve: 1) the failure to identify the
cause
and take prompt corrective action for a malfunctioning valve and for out
of specification water chemistry
and
2) the failure to follow procedures
for a.
valve lineup by the valve positioner,
the position verifier, and the shift
foreman.
The violations are of concern
because
they were identified by the
NRC and,
although the involved systems
were determined
to have remained
the violations represented
si gnificant failures to correct
conditions
adverse
to quality and should
have
been
prevented
by your
corrective actions for previous recent violations regarding failure to follow
procedures.

specified in the enclosed
Notice when preparing your response.
In your
response,
you shou'Id document
the specific actions
taken
and any additional
actions
you plan to prevent recurrence.
After reviewing your response
to this
Notice, including your proposed corrective actions
and the results of future
inspections,
the
NRC will determine
whether further
NRC enforcement
action is
necessary
to ensure
compliance with NRC regulatory requirements.
In accordance
with 10 CFR 2.790 of the NRC's "Rules of Practice,"
a copy of
this letter and its enclosures will be placed in the
NRC Public Document
Room.
The responses
directed
by this letter and the enclosed
Notice are not subject
to the clearance
procedures
of the Office of Hanagement
and Budget
as required
. by the Paperwork
Reduction Act of 1980,
Pub.
L. No. 96.511.
