This letter refers to the inspection
conducted
by Mr.
L.
C.
Carson II on
October
7 - 11,
and 29, 1991, of activities authorized
by
NRC License
No.
Discussions
of findings were held by the inspector with
Mr. J.
Baker and other members of the
WNP-2 staff at the conclusion of the
inspection.
Areas
examined during this inspection
are described
in the enclosed
inspection
report.
Within these
areas,
the inspection consisted
of selective
examinations
of procedures
and representative
records,
interviews with
personnel
and observations
by the inspector.
Based
on the results of this inspection,
certain of your activities appear to
be in violation of NRC requirements,
as specified in the enclosed
Notice of
Violation (Notice).
The violation of 10
CFR 20.201 resulted in the
third suspension,
in 18 months, of your Low Level Radioactive
Maste Disposal
Site disposal privileges
by the State of Washington
on October 9, 1991.
This
violation is of concern
because it indicates
a continuing programmatic
weakness
in the control of radioactive material
shipments that could lead to
more serious safety issues.
The second violation involved not following
established
procedures,
as required
by your Technical Specification 6.8. 1.
This violation is of concern
because it indicated that
some personnel
did not
implement your management
policy related to procedural
adherence.
You are requi red to respond to this letter and should follow the instructions
specified in the enclosed
Notice when preparinq your response.
In your
response,
you should document the specific actions
taken
and any additional
actions you plan to prevent recurrence.
After reviewing your response
to this
Notice, including your proposed corrective actions
and the results of future
inspections,
the
NRC will determine
whether fur'ther enforcement action is
necessary
to ensure
compliance with regulatory requirements.
The responses
directed
by this letter and the enclosed
Notice are not subject
to the clearance
procedures
of the Office of Management
and Budget as required
by the Paperwork
Reduction Act of 1980, Public
Law No.96-511.
