This refers to the routine inspection
conducted
by K. E. Johnston of this
office on July 15 through 19,
1991
and August
12 through 16,
1991 of
activities authorized
by NRC Licensee
No. NPF-21
and to the discussion of
findings held by the inspector with Mr. J.
W. Baker and other members of the
Supply System staff at the conclusion of the inspection.
Areas examined during this inspection
are described
in the enclosed
inspection
report.
Within these
areas,
the inspection consisted
of selective
examinations of procedures
and representative
records,
interviews with
personnel,
and observations
by the inspector.
Based
on the results of this inspection,
one of your activities appears
to be
in violation of NRC requirements,
as specified in the enclosed
Notice of
Violation (Notice).
The violation concerns
the failure to initiate
a Problem
Evaluation Request to address
high diesel starting air system pressures.
The findings discussed
in this report indicated
weaknesses
in the
implementation of the system engineering
program.
Although the program
expectations
appear consistent with those typical to the industry, the program
elements
necessary
for an engineer to implement these expectations,
such
as
training, procedural
requirements,
and resources,
have not been consistently
provided.
As a result, routine tasks
such
as
system walkdowns
and the
identification and review of emergent
problems are not always accomplished.
Problems
such
as the
one described
in the Notice
seem indicati,ve of the
reduced effectiveness
of the system engineers.
As discussed
with your staff
at the exit meeting,
these
issues
warrant further management
attention.
Your are required to respond to this letter and should follow the instructions
in the enclosed
Notice when preparing your response.
In your response,
you
should
document the specific actions
taken
and any additional actions you plan
to prevent recurrence.
After reviewing your response
to this Notice,
including your
proposed corrective actions
and the results of future
inspections,
the
NRC will determine
whether further
NRC enforcement action is
necessary
to ensure
compliance with NRC regulatory requirements.
