of this office on November

findings held by the inspectors with Mr.
C.
Powers
and other members of the
Supply System's staff at the conclusion of the inspection.
During this inspection,
several
concerns
developed
which you should address
in
response
to the enclosed
We are concerned
about the
apparent
lack of effective corrective actions in dealing with the torquing of
'asteners
used in applications that experience significant vibration.
This is
a known phenomenon
in the industry,
and
has occurred previously at WNP-2.
Your actions to date
have been insufficient to adequately
address this
problem, in that periodic preventive maintenance
examinations
did not have
acceptance criteria or receive engineering
review.
The method
used to select
proper torques for safety related
components
also appears
to be weak in that
the selection
process
is not reviewed
and is performed without engineering
input.
Areas
examined during this inspection
are described
in the enclosed
inspection
report.
Within .these
areas,
the inspection consisted of selective
examinations of procedures
and representative
records,
interviews with
personnel
and observations
by the inspectors.
Based
on the results of this inspection, it appears
that certain of your
activities were not conducted in full compliance with NRC requirements
as set
forth in the Notice of Violation, enclosed
herewith as Appendix A.
One
Severity Level
V violation was also identified,
as discussed
in paragraph
7.c
of the enclosed
report.
In view of the program improvements
already
implemented to address this concern,
and pursuant to 10 CFR Part 2, Appendix
C, this item is not included in the enclosed
Accordingly, a response
to this item is not required.
Your response
to this Notice is to be submitted in accordance
with the
provisions of 10 CFR 2.201 as stated in Appendix A, Notice of Violation.
