ENFORCEMENT CONFERENCE/MANAGEMENT MEETING
Gentlemen:
This refers to the enforcement
conference
and management
meeting held with
Mr. A. L. Oxsen,
Dr.
G.
D. Bouchey,
Mr. J.
P. Burn, and other
members of the
Supply System staff in the Region
V office on June 28,
1989.
The subjects
discussed
are
summarized
in the enclosed
report.
The purpose of the conference
was to discuss
potential
enforcement
issues
related to environmental qualification of electrical
equipment
and your
commercial
grade
procurement
and dedication
process.
After completion of the
enforcement
conference,
we also discussed
the high number of shutdown cooling
isolations that occurred during the
1989 refueling outage,
as well as other
issues of current interest.
Enforcement action involving the environmental qualification issues
discussed
during the enforcement
conference
is being reviewed
and will be addressed
in
separate
correspondence.
With regard to the commercial
grade
procurement
and dedication
process,
we
require
more information on the acceptability of the
10 items in question
before
we can conclude
whether
a hardware deficiency exists.
During the
conference
we agreed with your plan to reevaluate
the suitability of these
items for safety related
use,
which was expected
to take 30-45 days to com-
plete.
However,
due to the potential safety significance of the five items
already installed in the plant, reevaluation of these
items
was requested
on
an expedited basis.
Your reevaluation of the installed items
was provided to
us in your letter dated July 10,
1989 which we reviewed
and concluded that
additional
information was
needed
to complete
our evaluation.
Therefore,
we
requested
in our letter dated July 14,
1989, that you resubmit your response
to our original request with emphasis
on identification and verification of
the applicable critical characteristics.
Based
upon our discussions
at the
conclusion of the enforcement
conference,
we understand
your reevaluation for
the remaining
items will be submitted in writing by August 15,
1989 along
with any available
followup information on the five installed items.

As also discussed
in the enclosed
report,
we are concerned
about the
number
of shutdown cooling isolations
experienced
at the
WNP-2 facility during the
1989 refueling outage.
While we recognize that these
individual events
were
of minimal safety significance,
they clearly indicate
a
need for improved
control of plant activities and/or
improved reliability of safety features
actuation
systems.
We encourage
you to give careful
management
attention to
these
events
and their underlying root causes.
In accordance
with 10 CFR 2.790(a),
a copy of this letter
and the enclosure
will be placed in the
NRC Public Document
Room.
