by the inspectors

Powers
and other members of the Supply System
staff at the conclusion of the inspection.
Areas
examined during this inspection
are described
in the enclosed
inspec-
tion report.
Within these
areas,
the inspection consisted
of selective
examinations
of procedures
and representative
records,
interviews with
personnel
and observations
by the inspectors.
We are concerned
about the time being required to ensure fully effective
establishment
and implementation of your root cause
analysis
(RCA) program.
In particular,
we emphasize
the importance of analyses
in which your staff
is self-critical of its
own performance.
Paragraph
3.e of the enclosed
report discusses
a number of concerns
associated
with your
RCA report for
the Harch ll, 1989 rod drift event.
You are requested
to address
these
concerns
in
a revised
RCA and advise
us within 30 days
from the date of
this letter as to the actions
you will take to ensure that future
are conducted
in a more self-critical manner.
No violations of NRC requirements
were identified within the scope of this
inspection.
In accordance
with 10 CFR 2.790(a),
a copy of this letter and the enclosure
will be placed in the
NRC Public Document
Room.
The response
requested
above is not subject to the clearance
procedures
of
the Office of Hanagement
and Budget
as required
by the Paperwork Reduction
Act of 1980,
