SUBJECT: NRC INSPECTION REPORT 50-397/98-15 AND NOTICE OF VIOLATION
Dear Mr. Parrish:
From July 13-31, 1998, the NRC conducted fire protection and safety system engineering
inspections at your Washington Nuclear Project-2 reactor facility. Additional in-office review of
information provided by your staff was conducted following completion of the onsite inspection.
The enclosed report presents the scope and results of these inspections.
The fire protection inspection included a review of your fire protection program and direct
observation of a fire drill. The safety system engineering inspection included an assessment
of
the capability of the high piessure core spray system and supporting systems.
On the basis of
the results of our review, the fire protection program was satisfactorily implemented, and the
.
high pressure core spray system was capable of performing its safety functions. We also
concluded that your engineering program was generally good and improving, although a
number of violations were identified indicating a continuing need to ensure the fidelityof design
bases documentation as discussed below. The NRC has determined that five violations of NRC
requirements occurred.
These violations are cited in the enclosed Notice of Violation (Notice)
and the circumstances surrounding them are described in detail in the subject inspection report.
The first violation involved two examples of failure to meet the requirements to 10 CFR Part 50,
Appendix B, Criterion III, "Design Control," that resulted in technical specification values which
were not consistent with the commitments in the associated
bases sections.
Specifically:
(1) the Mode 4 and 5 surveillance requirement for condensate
storage tank level did not assure
the committed reserve volume of 135,000 gallons and (2) post-accident environmental effects
were not adequately considered when deriving the allowed value for reactor vessel water
Level 1 from the analytical limit. The second violation involved three examples of failure to
meet the requirements of 10 CFR Part 50, Appendix B, Criterion V, "Instructions, Procedures,
and Drawings."
Specifically: (1) the instructions for highlighting Regulatory Guide 1.97,
Category
1 instruments were not followed and instructions were not developed for highlighting
Regulatory Guide 1.97, Category 2 instruments;
(2) in conflict with the procedures, the
electrical short circuit analyses were prepared with nonconservative temperature assumptions;
and
(3) the cumulative impact of calculation modification records was not evaluated as
described in your procedure.
Please note that you are required to respond to this letter for
these violations. You should follow the instructions specified in the enclosed Notice when
preparing your response for these violations. The NRC will use your response,
in part, to
determine whether further enforcement action is necessary to ensure compliance with
regulatory requirements.

Based on the results of this inspection, three additional violations were identified that do not
require formal response:
(1) the Division 2 battery service test was not performed within the
frequency requirements of Technical Specification 3.0.2;
(2) the high pressure core spray
diesel generator thermal performance test acceptance
criterion was not valid for all allowed
high pressure core spray standby service water flows in violation of 10 CFR Part 50,
Appendix B, Criterion XI, "Test Control;" and (3) a longstanding design error related to electrical
protection coordination was not corrected in a,time frame commensurate
with its safety
significance in violation of 10 CFR Part 50, Appendix B, Criterion XVI,"Corrective Action." With
respect to these violations, the NRC has concluded that information regarding the reason for
these violations and the corrective actions taken and planned to correct the violations and to
prevent recurrence is already adequately addressed
on the docket in the enclosed inspection
report. Therefore, you are not required to respond to this letter, unless the description therein
does not accurately reflect your corrective actions or your position.
In that case, or if you
choose to provide additional information, you should followthe instructions specified in the
enclosed Notice.
One unresolved item was identified concerning establishment of appropriate leak testing for
liquid secondary containment bypass valves. This matter is discussed
in Section E2.3 of the
enclosed inspection report and is unresolved pending NRC review of the calculation method for
evaluating the consequences
of leakage from the liquid bypass isolation valves.

One last issue was noted, which warrants additional management attention. The issue is your
acknowledgment of inadequate progress on Thermo-lag barrier modifications to meet
Confirmatory Order dates.
You are encouraged
to address this area to ensure that the
Confirmatory Order dates are met.
In accordance with 10 CFR 2.790 of the NRC's "Rules of Practice," a copy of this letter, its
enclosure(s), and your response
will be placed in the NRC Public Document Room (PDR).
