SUBJECT: NRC INSPECTION REPORT 50-397/98-13 AND NOTICE OF VIOLATION
Dear Mr. Parrish:
From June 7 through July 18, 1998, an inspection was conducted at your Washington Nuclear
Project-2 (WNP-2) reactor facility. The enclosed report presents the scope and results of that
inspection.
The inspection period was highlighted by plant restart from WNP-2's 13th refueling outage, a
subsequent
plant shutdown to repair the traversing incore probe system, and the June 17 fire
main flooding event.
In general, your staff responded well to each of these challenges with the
proper focus on operational safety and appropriate identification and resolution of the
underlying issues.
ALARAplanning and execution of repairs to the traversing incore probe
system was noteworthy.
However, in the course of our review of these events, two violations of
NRC requirements were identified. The first violation is related to the mispositioning of a low
pressure core spray system valve. We note that subsequent to the valve being mispositioned
numerous control room board walkdowns failed to identify the discrepancy.
The second
violation is related to the failure to install a cap on the equipment drain line from residual heat
removal pump Room B. This cap was a barrier against common mode flooding in the reactor
building, as described in the WNP-2 Final Safety Analysis Report.

These violations are cited in the enclosed Notice of Violation (Notice) and the circumstances
surrounding these violations are described in detail in the enclosed report. The NRC has
concluded that information regarding the reasons for the violations, and the corrective actions
taken and planned to correct the violations to prevent recurrence, is already adequately
addressed
on the docket in the enclosed report. Therefore, you are not required to respond to
this letter unless the description therein does not accurately reflect your corrective actions or
your position.
In that case, or ifyou choose to respond, you should followthe instructions
specified in the enclosed Notice.
