This refers to the
NRC's Systematic
Assessment
of Licensee
Performance
(SALP)
Board Report for Washington Nuclear Plant
(WNP-2) dated July 20,
1988; our
meeting of August 18,
1988, in which we discussed
the contents of the Report;
and your written comments
in response
to the Report dated
September
17,
1988.
During the August 18,
1988 meeting,
we discussed
your planned corrective actions
relating to this assessment
of your activities authorized
by NRC License
Subjects
discussed
during the meeting are
summarized
in the enclosed
inspection report 50-397/88-31.
The
NRC had
a number of concerns
with Supply System performance during the
period.
These
concerns
received special
emphasis
in our discussions
during
this meeting
and were as follows:
Yianagement
involvement in activities (e.g., interface with personnel
to
convey expectations
on performance etc.)
Insufficient follow-through/timeliness
on commitments
and corrective
actions
Personnel
performance
error s resulting from insufficient operator
attention to procedures
or proceeding
in the face of uncertainty,
particularly in the operations
area
Inadequate
preparation of equipment clearances,
resulting in a number of
near misses
Engineering/technical
support effectiveness
Involvement and effectiveness
of quality oversight groups

We reviewed your response
to the
WNP-2 SALP Board Report,
dated
September
17,
1988,
and noted that there
were
no comments
addressing
the specific content of
the report.
We feel that your proposed corrective actions to deal with areas
needing
improvement appear to be responsive
and substantial..
As discussed
by
D. F. Kirsch, of my staff, and A. L. Oxsen, of your staff, by telephone
on
October 6, 1988,
we note that your plans for improvement could benefit from the
application of a systematic
program to periodically assess
the effectiveness
of
your initiatives to improve performance
and provide any necessary
corrective
adjustments.
Accordingly, please
provide us with your plans for periodically
assessing
the effectiveness
of your corrective action.
We will discuss
the
results of your periodic assessment
during .each of our future management
meetings.
Based
on the formal exchange of information between our respective staffs,
and
in the absence
of verbal identification of discrepancies
within the report
or formal written comments
from you requiring resolution,
no changes
to the
SALP Board Report are necessary.
In accordance
with 10 CFR 2.790(a),
a copy of this letter and the enclosures
will be placed in the
NRC Public Document
Room.
No reply to this letter is required;
however,
should you have
any questions
concerning the
SALP Report, Appendix thereto, of our SALP meeting,
we will be
pleased to discuss
them with you.
