The

NRC Systematic

Assessment

of Licensee

Performance

(SALP) Board has

completed

its periodic evaluation of the performance of your Washington

Nuclear Plant

Number

2

(WNP-2) facility during the period of June

1,

1987 through

May 31,

1988.

The performance of WNP-2 was evaluated

in the functional areas of plant operations,

radiological controls, maintenance/surveillance,

emergency

preparedness,

security,

engineering/technical

support,

and safety assessment/quality

verification.

The

criteria used in conducting this assessment

and the

SALP Board's evaluation of your

performance

in these functional areas

are contained

in the enclosed

SALP report.

A management

meeting to discuss

the results of the

SALP Board's

assessment

is

tentatively scheduled for August 18,

1988.

This date

and the location for the

management

meeting will be discussed

with you in the near future.

Overall, the

SALP Board found your performance

to be acceptable

and directed

toward

safe facility operatiop.

However, there

were

a number of concerns

identified

by the

NRC during this assessment

period.

Concerns

common to the functional areas

assessed

were insufficient Supply System

management

involvement in activities

and

insufficient follow-through on commitments

and corrective actions.

Weaknesses

were

identified that were specific to four key functional

areas

as follows:

Operations - the

number

and type of personnel

errors resulting from insufficient

operator attention to plant procedures,

proceeding in the face'of uncertainty,

and inadequate

preparation of equipment clearances.

Maintenance/Surveillance

- maintenance

personnel

not following procedures

and

insufficient controls over work performed

on vital maintenance

work requests.

Engineering/Technical

Support - insufficient understanding

of plant design,

inadequate

control of design

processes,

and discrepancies

in the design

data

base.

Safety Assessment/guality

Verification - the ineffectiveness

of quality groups

in identifying developing plant problems,

slowness

and ineffectiveness

in

implementation of the root cause

assessment

program,

and marginal staffing in

various groups.

Your performance

in plant operations

and engineering/technical

support

was evaluated

Category 3.

You are requested

to provide

a written response

within 30 days after

our forthcoming meeting which addresses

your plans to improve performance

in these

functional areas.

Comments

on other portions of the

SALP report may be provided

as appropriate.

weaknesses,

2.790 of the NRC's "Rules of Practice,"

Code of Federal

Regulations,

a copy of this letter and the enclosed

SALP report will be placed in the NRC's Public Document

Room,

as well as

any

comments

you may wish to submit to

NRC regarding

the content of the

SALP

report.

The NRC's Office for Analysis and Evaluation of Operational

Data performed

an

assessment

of licensee

event reports

submitted for WNP-2.

This assessment

was

provided

as

an input to the

SALP process;

a copy is therefore

provided

as

Attachment I to the enclosed

report.
