CPPR-93 NOTICE OF VIOLATION 0

As a result of the inspection conducted on February 2-27,

1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980), the following violation was identified:

10 CFR 50, Appendix B, Criterion V, as described in Appendix D of your PSAR and as addressed in Paragraph D.2.5.5 of the gA program states in part: "activities affecting quality:..shall be accomplished in accordance with these. instructions, procedures, or drawings...."

The WBG procedure gAP-211 Revision 0 "NDE for-Hag. Particle Examination" part 4-5-1 requires that:

"The results of each inspection shall be re-corded on the appropriate Bovee and Crail form:...the inspection report form NF-23B including a rollout drawing if required shall be used."

Contrary to the above, on February 13, 1981 NIX magnetic particle in-spection personnel failed to record the inspection results (type of indication) which revealed existence of a partial penetration weld where a full penetration weld was designated on Leckenby design drawing k'66 for the sacrificial shield wall.

This is a severity level V violation (Supplement II of Federal Register Notice 45 FR 66754, dated October 7, 1980).

Pursuant to the provisions of 10 CFR 2.201, Washington Public Power Supply System is hereby required to submit to this office within twenty-five days of the date of this Notice, a written statement or explanation in reply in-cluding: (1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further items of noncompliance; and (3) the date when full compliance will be achieved.
