SUBJECT: SYSTEMATIC ASSESSMENT OF LICENSEE PERFORMANCE (SALP)

REPORT NUMBER 50-244/92-99

This letter forwards the SALP report for the R.E. Ginna Nuclear Power Plant for the period

January

19, 1992, through September

11, 1993.

This SALP was conducted under the

revised SALP process

that was implemented by the Nuclear Regulatory Commission (NRC)

on July 19, 1993.

The revised SALP process rates the performance of licensees in four

functional areas:

operations,

engineering,

maintenance,

and plant support.

The plant

support area includes:

radiological controls, emergency preparedness,

security, fire

protection, chemistry, and housekeeping

controls.

Generally, improved performance at the Ginna plant continued.

During the SALP period,

both corporate and plant management

demonstrated

a proactive approach in dealing with

safety issues,

and provided excellent oversight, control, and support of plant activities.

-Significantly improved performance and management

effectiveness were demonstrated

in the

engineering area.

The offsite and onsite review committees'oversight

was notably effective.

Staff training and performance in problem identification and root-cause analyses

also were

notable strengths.

While your performance in radiological controls was considered

to be good, weaknesses

were

noted in revising radiological controls procedures

and in implementing the ALARA

and'adiological

environmental monitoring programs.

During the next few years, there willbe a

significant challenge associated

with the planned replacement of the steam generators.

This

activity will require substantial

increases

in resource demands and new challenges

to your

radiological controls program.

The special attention that you will devote to this activity

while attaining improved performance in the area of radiological controls willbe a significant

management

challenge.

Activities associated

with staff training and development were outstanding.

These included

the lateial reassignments

of staff from the Operations Department and the participation in

visits to other sites with two-loop Westinghouse power plants.

Performance in the operations area remained a Category 2.

Corporate and plant management

continued to exercise good oversight of plant operations.

Operators were frequently

challenged by equipment problems in the balance of plant and responded

well to the

associated

plant transients and abnormal conditions.

~ However,.weaknesses

were evident in

the control of'ysteni configurations and the initial management

response

was inadequate in

followup to the significant industry issue concerning falsification of logs by operators.

Performance in engineering improved from Category 2 to Category I. Strong management

oversight was exhibited for planning and implementation of major modifications.

Communications were outstanding between the corporate and site staffs.

However,

inadequate corrective action for the failed valves in the service water system could have been

avoided had the earlier failures in similar valves been properly evaluated.

Performance in maintenance

remained

a Category

1. A quality-oriented maintenance

environment existed at Ginna, and improvements to maintenance activities continued to be

aggressively pursued.

However, increased

management

attention is needed to reduce the

frequency of challenges

to both safety systems and operators,

caused by secondary

system

equipment problems.

Performance in the new area of plant support was rated Category 2.

The overall

performance

was considered

to be good, with superior performance in the security program

being a notable highlight. Performance during the emergency preparedness

drill continued to

improve and excellent rapport continued with offsite organizations.

Areas needing improved

performance were noted in the radiological controls, environmental monitoring,

housekeeping,

and fire protection programs.

We have scheduled

a management

meeting with you that will be open for public observation

at I:00 p.m. on November 15, 1993, at the Ginna Training Center.

At the meeting, you

should be prepared

to discuss our assessment

and any initiatives you plan to take to address

the weaknesses

identified in the SALP report.
