NRC BULLETIN 89-01, SUPPLEMENT 2, "FAILURE OF WESTINGHOUSE STEAM GENERATOR TUBE MECHANICAL PLUGS" - R.

GINNA NUCLEAR POWER PLANT (TAC NO. M73172)

On June 28, 1991, NRC Bulletin 89-01; Supplement 2 was issued to all holders of operating licenses or construction permits for pressurized water reactors (PWRs).

The bulletin requested that actions similar to those requested in NRC Bulletin 89-01, "Failure of Westinghouse Steam Generator Tube Mechanical Plugs,"

be extended to include all Westinghouse mechanical plugs fabricated from thermally treated Inconel 600.

These actions were necessary to ensure that these plugs would continue to provide adequate assurance for reactor coolant system (RCS) pressure boundary integrity under normal operating, transient, and postulated accident conditions.

Your response to Bulletin 89-01, of June 21, 1989, verified a review of information provided by Westinghouse in WCAP-12244, Revision 1, and Reference 2

of Bulletin 89-01.

We understand from your response letter of June 21, 1989, that no Inconel 600 mechanical plugs from heat batches,

3279, 3513,
3962, and 4523 (group 1 heats) are installed in the Ginna steam generators (SG's).

No response was required to Bulletin 89-01, Supplement 1, of November 14, 1990, which identified reports of instances of primary water stress corrosion cracking (PI'SCC) at other plants affecting other heats (group 2 heats).

Bulletin 89-01, Supplement 2, of July 29, 1991, requested similar actions to that of Bulletin 89-01 for group 2 heats identified in Westinghouse WCAP-12244, Revision 3, Addendum 2.

As you reported, out of a total of 176 Westinghouse mechanical Inconel 600 plugs still installed, only 89 have life expectancy less than the full 40-year plant life (2009),

and are located in SG No.

Forty Inconel 600 plugs must be removed by the 1996 refueling outage (RO) and 49 plugs by the 1998 RO.

These plug lifetimes have been determined by your analysis using actual cycle burn-up data and estimating the remaining plug life in effective full power days (EFPDs).

'r. Robert C. Mecredy November 27, 1991 Because the correlation for SG mechanical plug operating life, which was developed by Westinghouse for various heats of plugs installed, differs from the EFPD operating life for plugs installed at Ginna, please submit your re-calculations and methodology.
