SPECIAL TEAMINSPECTION REPORT No. 50-244/90-31 (12/12/90- 1/3/91)
During the subject period, members of the NRC Region I staff conducted
a special safety
inspection at the Robert E. Ginna Nuclear Power Plant as a result of a safety significant event
on December 12, 1990. We documented the results ofthose inspection findings in the enclosed
report.
At the conclusion of the inspection period, Mr. R. Conte summarized
the inspection
findings with.you and other members. of.your staff.
The event involved the disabling of safeguards logic instrumentation for about twenty minutes
during the conduct of a maintenance
activity.
The event was as a result of an inadequate
maintenance procedure coupled'with the random equipment failure generating the need to use
that procedure.
Your organiRtionSaled;& detect the inadequate procedure and its improper
implementation which resulted in a safety significant event.
We are concerned about the proper
implementation of your Technical and Safety Review Program that permitted the inadequate
procedure
to be
issued.
However,
we are
equally
concerned
about
the program
and
implementation inadequacies
and weaknesses
noted by the team which contributed to the adverse
sequence of events that led to this event.
Further, based on the results of this inspection, apparent violations of NRC requirements
are
identified and are being considered for escalated
enforcement action in accordance
with the
"General Statement of Policy and Procedure for NRC Enforcement Actions" (Enforcement
Policy), 10 CFR Part 2, Appendix C.
See section 3.2 of the enclosed report.
No Notice of
Violation is presently being issued for these inspection findings. In addition, please be advised
that the number
and characterization
of the apparent
violations described
in the enclosed
inspection report may change as a result of further NRC review.

An enforcement conference to discuss these apparent violations has been scheduled for 10:00 am
on January 29, 1991.
The purposes of this conference are to discuss the apparent violations,
their causes and safety significance; to provide you the opportunity to point out any errors in our
inspection report; to provide an opportunity for you to present your proposed corrective actions;
and to discuss any other information that willhelp us determine the appropriate enforcement
action in accordance with the Enforcement Policy.
Also, during the conference,
you should be prepared to discuss the additional deficiencies and
weaknesses
noted in the attached
report.
With respect
to those findings, we expect you to
address
the
adequacy
of your long
term
actions.
You will be
advised
by
separate
correspondence of the results of our deliberations on this matter.
No response regarding these
apparent violations is required at this time.
In accordance with 10 CFR 2.790 of the NRC's "Rules of Practice," a copy of this letter and
its enclosure will be placed in the NRC Public Document Room.
