During the period December
11-21,
1988 and January
4,
1989,
members of NRC
Region I staff conducted
a special
inspection of the December ll, 1988 frozen
B
pressure
sensing
lines,
and actions
taken to prevent
a Safety
Injection Signal.
At the conclusion of the inspection period Messrs.
C. Cowgill, C. Marschall,
and
N. Perry of my staff summarized
inspection findings
with you and
members of your staff.
The event placed
the plant in a condition requiring the operators
to act in a
highly unusual
way to avoid
an unwanted plant transient.
Our inspection
iden-
tified "two potentially significant safety issues."
One problem was the failure
to take adequate
corrective action for the
known problem of pressure
sensing
line freezing.
The other
problem involves the insertion of a simulated signal
into plant protective circuits to prevent the unwanted automatic actuation.
Since there are apparent violations of NRC regulatory requirements
involved,
we
will contact you shortly to schedule
an enforcement
conference
in the Region I
offices to discuss
these matters.
You should
be prepared
to discuss
the appro-
priateness
of the actions
taken
by licensed operators
in response
to the event,
the root causes
for these
problems
and your short
and long term actions to
correct the current problems
and preclude similar problems
in the future..
In accordance
with 10 CFR 2,790,
a copy of this letter and enclosures will be
placed in the
NRC Public Document
Room.
