on the results of this inspection, it appears

activities was not in full compliance with NRC regulations
as set forth in the

Notice of Violation, Appendix A of this report.

is required to be submitted in accordance
with the provisions of 10 CFR 2.201.
In accordance
with 10 CFR 2.790,

placed in the
