The f l ood seal covers and support e l ements f or Aux. Feed Tunnel s (1/2DSFS006/07/08/09) do not meet the design bas i s requi rements f or their as i nstall ed conf igurations for Hi gh Energy Line Break (HELB) loadi ng.

This condi t i on has been identifi ed in I R' s 653093 and 620080.

Fur ther eval uation as documented i n EC 366723 (Attachment - A) i ndicates that a HELB pressure of 17. 7 ps i on the s ubject covers and supporting e l ements woul d be acceptabl e for the system Operability Evaluati on.

This condi t i on i s applicable to a l l o f t he covers f or t he Aux. Feed t unnel s as incl uded i n the AEL o f t his EC.

Revis i on 1 shows CA ' s #1 and 2 as COMPLETE and adds CA ' s #3 through 10.

The new CA ' s will track compl eti on of EC ' s f or each MSI V Rooms flood seal s and completion of the WO ' s to modi fy t hem per the EC' s.

There are no changes to the oper ability determi nation or engi neeri ng support.

Revis i on 2 shows CA ' s #3 and 4 as COMPLETE and revi ses the due dates f or CA ' s #7 and 8 for i nstal lati on o f U-1 t o 06/06/08.

Install ati on o f U-1 was p l anned f or the f i r st week o f May, 2008, but was not perf ormed due to concerns that the work could not be compl eted wi thi n the allowed LCOAR time and may have to be moved into outage B1R16.

I R 764527 was wri t ten documenting these concerns, and ATI 764527- 02 t o Des i gn Engi neeri ng i s tracking resolution, due 05/30/ 08.

Therefore, i t i s expected that CA ' s #7 and 8 may requ i re extension agai n once these concerns have been worked out and new installation dates can be determined.

Revis i on 3 addresses the impact of an i ssue (I R 789344, Ref 2. 5. 3. 13) associ ated with t he di scovery t hat p l ates i nstal l ed next t o the MSIV room venti l ation fan discharge damper s, that were assumed to be blow-out plates by the pressuri zati on cal culation, were i n f act blank-off

plates, not b l ow-out panels.

The vent area assumed in Reference 2. 5. 3. 10 was 51. 3 f t2 per MSI V room.

Further revi ew by Engineeri ng resul ted i n I R 790 428 (Ref 2. 5. 3. 18) t hat documents the potential for i nsuffi cient mar g i n wi th respect to AF FSO structural integrity.

New FSO covers will be i nstalled per the CA ' s currently ass i gned, using a bounding pressure assumption.

CA # 11 created several i nteri m ACI Ts to compl ete a new mass and energy release anal ys i s and a steam tunnel p ressurizat i on anal ys i s.

I t a l so c reated a new CA #12 to verif y that the new FSO cover des i gn has rest ored t he required fac t ors o f safety pri or t o c l os i ng thi s Op Eval.

Revis i on 5 extends t he due dates o f CA ' s #7, 8, 9, and 10 to 03/31/09 and CA #12 to 04/10/09.

I t also adds a requirement t o CA #12 to determi ne i f the Blow-out panel TCCP ' s can be removed or must be made permanent.

There are no technical changes.

The most recent p l an had been t o perf orm the Uni t 2 modif i cat i ons i n B2R14, and the Unit 1 modi fications the f oll owi ng month.

However, a management deci sion was made t o not perf orm the Uni t 2 work as or i ginal l y p l anned due t o resources.

The work was SCRF ' d out of B2R1 4.

Revi s i ons requi red to the mod EC' s are not compl eted to support the work.

This i s due in part t o NRC Generi c Letter 08- 01 on gas voi ding concerns.

There is i nformation as well that the EC' s will provi de a product that will not chall enge LCO t i me to perform the work on l ine.

The fir s t oppor tuni ty to perform thi s work i s i n March 2009.

MRC approved t h i s extens i on on 9/30/08.

Revis i on 6 shows CA ' s 7, 8, 9, 10, and Comp Action #4 as Compl ete.

These acti ons installed the final AF tunnel cover s and TCCP ' s to change the b l ank-off p l ates t o b l ow-out panel s.

I t adds CA #13, whi ch wi l l t rack maki ng the Blowout panel TCCP ' s permanent modifi cations.

Complet i on of this CA wi ll all ow c l osure of thi s Op Eval.

Revis i on 7 shows CA ' s 12 and 13 as compl ete.

I t a l so removes the "Compl ete to Close EC" checkmark i n Passpor t for TCCP EC 371279.

TCCP EC 371279 installed temporary AF tunnel covers.

To i nstall some o f t he braci ng supporti ng the temporary cover i nstallati on, a sound-powered phone condui t was cut.

The temporary cover and braci ng has been phys i cal ly removed, but EC 371279 is remaini ng open because the condui t has not yet been repai red or permanentl y accepted as-is.

EC 367211 has been i niti ated and associ ated t o EC 371279 to repair the condui t or accept i t ' s current condi t i on permanentl y. Because t he conduit i s appropri ately seal ed, i t does not i mpact the abi lity of t he AF Tunnel Covers t o perf orm their function, and thus EC 371279 is not required to be compl eted in or der to close this Op Eval.

The concern is t hat the covers and supporti ng e l ements do not meet the des i gn basi s for HELB l oadi ng.

However, it does meet the operability requi rement for the HELB l oading, whi ch is determined to be an acceptabl e l oadi ng, whi ch i s documented in EC 366723 (Attachment-A)

The i nappropriate assumpti on (addressed by Rev 3 o f this Op Eval ) that the blank off p l ates relieve pressure woul d result i n h i gher MS I V r oom pressure potenti ally making the AF FSO i noperabl e.

(b) any requi rement s or commi tment s established for the SSC and any challenges to these ;

The covers are designed to stay i n p l ace for des i gn l oading and p r otect the tunnel f rom f loodi ng or any other harsh envi r onment.

Based on desi gn calcul ations, the support members for the cover do not meet the design bas i s requi rements f or HELB loadi ng, however, the operabili ty c r i teri a f or the f a i ling component of the cover support i s met, whi ch are the concrete expansi on anchors that mounts the cover plate support members on t he concrete wal l.

This evaluation showi ng operability acceptance i s contained i n EC 366723 (Attachment-A).

(c) the circumstances o f the degraded/nonconf orming condi tion, i ncl udi ng the possibl e failure mechani sm(s) ;

The possibl e failure mechani sms i s overl oad of the p l ate or suppor ting elements.

This woul d allow a s team water mi xture to flow t o the area containing the AF013s.

Al t hough the val ves are of the same type and have similar mai ntenance as envi ronmentall y qualified valves, the AF013s are not environmental ly qualified.

Theref ore t he harsh envi ronment could cause the valves to f a i l.

There are current l y n i ne other open Op Eval s that i mpact Byron Uni t 1 and 2.

However, none of these are associated wi t h the Aux. Feed Tunnel cover, and t here is no adverse aggregate effect o f these ni ne open Op Eval s on t h i s issue, or v i ce-versa.

2. 3 I s SSC operability supported?

Explai n bas i s YES (e. g., analysis, test, operati ng experi ence, engi neering j udgment, etc. ) :

EC 366723 (Attachment-A) contains the evaluati on t hat shows the cover support members woul d meet the operability criteria f or revi sions of t h i s Op Eval through Rev 3.

To esti mate the i mpact of t he reducti on in vent area i n Rev 3 o f t he Op Eval, Sargent and Lundy (S&L) was contacted to perf orm study runs on each unit.

These study r uns had the f ollowi ng attri butes :

The sof tware program used (RELAP) i s accepted by the i ndustry for this type o f anal ys i s and i s the same software used i n Reference 2. 5. 3. 10,

S&L has an approved QA program and the sof tware was run from a contro l led l ocation, The runs were performed by a qualified anal yst wi th oversight from a seni or engi neer, Inputs were devel oped with i nput f rom Byron, Brai dwood and Corporate Engi neering.

The f o l lowi ng are the signi ficant changes t o the model used i n Ref erence 2. 5. 3. 10 :

Credi t was taken f or more realisti c mass and energy rel ease, (Ref 2. 5. 3. 21)

The vent paths f r om the MSI V room to t he Mai n Steam Tunnel were more accurately model ed (pressure penalty),

Credi t was taken f or t he openings created under References 2. 5. 3. 14 and 2. 5. 3. 15.

Credi t was taken f or t he annular flow area between the MSIV f an motor and f an shroud.

Note that no credit was taken f or the fan runni ng.

Security bars were modeled as parti al f low obstr uction.

Credi t was taken f or f l ow path of normally open dampers on f an discharge (after adjusting for the obstruction due t o thickness of the damper blades).

The model was run in an iterative manner where Byron and Br a i dwood Engi neering discussed the effects o f the changes f or i mpact and reasonableness wi th S&L.

Based on t he S&L study runs, the peak pressure i n the MSIV room a f ter accounting f or t he above model changes was 21. 7 psi g f or Uni t 1 and 19. 4 psig f or Uni t 2.

Since the pressures have changed, new dynami c load factors were determi ned by S&L and p r ovided in Reference

Note that work i s in progress t o instal l sti ffeners on the AF FSO p l ates and that the DLFs onl y represent the AF FSO plates pri or t o the i nstallati on of t he s tiffeners.

On Uni t 1, two Temporary Configuration Changes (TCC) were i nstalled t o enhance a degraded but operable condi t i on ; removal o f the b l ank o f f p l ates (Ref 2. 5. 3. 15) and s trengthening of the AF FSO covers (Ref 2. 5. 3. 1 6).

The design of the s t iffener TCC on Uni t 1 used a peak pressure o f 20 psi g and DLF o f 2. 0 (Reference 2. 5. 3. 20).

As the TCC was bei ng i nstalled, Sargent and Lundy provi ded additi onal i nformati on that the peak p ressure was 21. 7 psi g, i nstead of t he 20 psig used f or the desi gn of the TCC.

(Thi s occurred due t o the parall e l paths o f refi ni ng t he anal ys i s and i nstalling the TCC. )

The calcul ation (Reference 2. 5. 3. 20) used t o devel op the f actors of safety f or t he TCC was reviewed; adequate margin exi s t s to compensate f or t he i ncreased pressure and still assure operabili ty o f the AF FSO Concrete Expansi on Anchors with a factor of saf ety greater than 2. 0. A fini te e l ement anal ysis of the p l ate itsel f shows that the plate woul d remai n intact (Reference 2. 5. 3. 17). Theref ore the Uni t 1 AF013s shoul d be considered operabl e wi th the enhanced margi n gained by t he two TCCs.

For Uni t 2 the b l owout panel TCC has been i nstalled, but Uni t 2 i s diff erent than Unit 1 i n two other aspects ; the tube steel stiff eners have not yet been install ed and the peak pressure in t he MSIV rooms are lower.

The pressures are lower due to different mass and energy rel eases resul t i ng f rom di fferences i n SG des i gn.

These offsett i ng factor s have been reviewed in Ref erence 2. 5. 3. 17 and the factor o f safety for t he CEAs i s 1. 35.

This i s suf fic i ently (but subj ectivel y) greater than 1 that operability is assured on Unit 2 by onl y c rediti ng the blowout panel TCC.

The sti ffener TCC is being i nstalled to increase margi n.
