1986 at
the
R.
E. Ginna Nuclear
Power Plant of activities authorized
by
NRC license
No.
DPR-18 and to the discussions
of our findings by Mr. Cook with Mr. S.
M. Sp'ector
and other members of your staff periodically during the inspection.
Areas
examined during this inspection
are described
in the
NRC Region I Inspec-
tion Report which is enclosed with this letter.
Within these
areas,
the in-
spection
consisted
of selective
examinations
of procedures
and representative
records,
interviews with personnel
and observations
by the inspectors.
Based on, the results of this inspection, it appears
that one of your activities
was not conducted
in full compliance with NRC requirements,
as set forth in the
Notice of Violation, enclosed
herewith as Appendix A.
Specifically,
on
February
16,
1986 you discovered that containment integrity requirements
for
refueling were violated while maintenance
was being performed, concurrently,
on Vital Bus 14.
Although self-identified,
and of relatively low safety
significance,
this event is considered
by us to be another
example of insuf-
ficient review, control
and planning of station maintenance activities as
identified and discussed
in our January
24,
1986 cover letter for Inspection
Report 50-244/85-26.
This violation has
been categorized
by severity level in accordance
with the
General
Statement
of Policy and Procedure
for Enforcement Actions (Enforce-
ment Policy),
10 CFR 2, Appendix
C (1985).
You are required to respond to this
letter and in preparing your response,
you should follow the instructions in
Appendix A.

As discussed
with you we believe that your February
21,
1986 response
to the
Inspection
Report 50-244/85-26 violation does not reflect an understanding
of
our concern.
It appears
that your administrative controls for plant main-
tenance
are not uniformly applied to ensure that all station
maintenance
activities are reviewed
and appropriately evaluated for their affect or poten-
tial impact on Technical Specification required
and safety-related
equipment.
In your response
to this second violation we request that you include actions
taken to address
the previous violation concerns
and steps
taken to prevent
recurrence.
The response
directed
by this letter and the accompanying
Notice are not
subject to clearance
procedures
of the Office of Management
and Budget as
required
by the Paperwork Reduction Act of, 1980,
PL 96-511.
