SUBJECT:
NOTICE OF VIOLATION(NRC INSPECTION REPORT NO. 50-244/93-10)
This letter transmits the April23 to June 9, 1993 inspection findings of Messrs. T. Moslak and
E. Knutson at the R. E. Ginna Nuclear Power Plant. At the end of the inspection, the findings
were discussed with Mr. J. Widay and other members of your staff.
The inspection primarilyexamined the control ofoperations and maintenance activities conducted
during this period.
Within this scope,
good performance by the operations department
was
observed during the reactor startup and power ascension,
steady state power operations,
and
power reduction to support "A"main feedwater pump maintenance.
The prompt identification
of a small, intermittent secondary plant leak by an on-shift operator, using the in-containment
video monitoring system, demonstrated
the attentiveness of operations department personnel.
Based on the results of this inspection, your fire protection program appeared to be in violation
of NRC requirements,
as specified in the enclosed Notice of Violation (Notice). The violation
is ofconcern because it was identified by the NRC and your failure to implement the appropriate
fire prevention measures while performing grinding operations could have compromised plant
safety.
You are required to respond to this letter and should follow the instructions specified in the
enclosed Notice when preparing your response.
In your response,
you should document the
specific actions taken and any additional actions you plan to prevent recurrence.
Afterreviewing
your response to this Notice, including your proposed corrective actions and the results offuture
inspections,
the NRC willdetermine whether further NRC enforcement action is necessary
to
ensure compliance with NRC regulatory requirements.
In accordance with 10 CFR 2.790 of the NRC's "Rules of Practice," a copy of this letter and
its enclosures willbe placed in the NRC Public Document Room.
The response
directed by this letter and the enclosed Notice are not subject to the clearance
procedures of the Office of Management and Budget as required by the Paperwork Reduction
Act of 1980, Public Law No. 96.511.
