REQUALIFICATION PROGRAM EVALUATION REPORT

On June

19

23,

1989,

the

NRC administered requalification examinations

to

employees

of your company

who operate

the

R.

E. Ginna Nuclear

Power Station.

The evaluation

was conducted

in accordance

with NUREG-1021, Operator

Licensing

Examiner Standards,

ES-601, "Administration of NRC Requalification

Program

Evaluations."

The evaluation

had two purposes,

evaluation of the effectiveness

of the licensed operator requalification training program at maintaining the

competency of the licensed operators,

and examination of the individual opera-

tors to determine if they would satisfy the regulatory requirement of passing

an

NRC administered

examination prior to license

renewal.

At the conclusion of

the examination/evaluation,

any generic findings that evolved

as

a result of

the examination

were discussed

with those

members of your staff identified in

the enclosed report.

As evaluated

by the

NRC, six of eleven operators

(one

SRO,

excused

from the

written portion of the examination,

completed

the examination during the week

of August 7,

1989) failed one or more

segments

of the examination.

As a result

of this evaluation,

your requalification

program

has

been

assigned

an overall

program rating of "unsatisfactory."

The successful

completion of the examina-

tion by the previously excused

SRO does not change

the program rating,

These

evaluations

were conducted

in accordance

with the criteria of NUREG 1021,

ES-601,

noted above.

The six operators

who failed the examination

were

immediately removed from shift operational

duties

and assigned

to an upgrade

training program designed

to address

each individual's specific needs.

Return

to licensed duties cannot occur unti l after sati sfactory reexamination

by the

NRC.

Following the completion of the examination,

a meeting

was held on June

30,

1989, at the

NRC Region

1 office in King of Prussia,

Pennsylvania

to discuss

the results of the examination

and various courses

of actions resulting from

the "unsatisfactory"

program evaluation.

Results of this meeting are detailed

in the enclosed

report.

By letter, dated July ll, 1989,

the licensee

provided to the

NRC a basis for

continued operation

as

a result of the unsatisfactory requalification program

evaluation.

This letter addressed

several

areas,

including the adequacy of the

crew which replaced'he

crew that failed the examination;

the adequacy of the

remaining crews

on shift; and,

the planned

implementation of the upgraded
